Urethritis or cystitis has been treated with medication, urine tests are normal, but discomfort persists?

Hello, doctor.
My situation is a bit complicated, and I appreciate your attention.
Recently, I was preparing for an exam and suddenly experienced frequent urination.
At first, I ignored it, but later I visited a family medicine clinic, and they diagnosed me with urethritis.
I started taking antibiotics for three days, then took a day off (due to the exam, I didn't have time to take the medication), and then continued for another three days (from 3/2 to 3/9).
After completing the six-day course, I felt normal.
About a week later, on 3/16, I felt the sensation of urine reaching the urethral opening again.
Since I was abroad at the time, I took the antibiotics prescribed by the previous clinic and continued taking them until today, 3/22.
On 3/21, I visited a urology department at a hospital, and the doctor assessed that it might not necessarily be urethritis but could be cystitis.
They immediately requested a urinalysis, and the results showed that my urine was clear, indicating no issues.
The doctor advised me to stop taking antibiotics.
Therefore, on 3/21, after taking antibiotics at lunch, I skipped the evening dose.
However, before bed, I experienced the sensation of urine reaching the urethral opening again, which felt like unusual frequent urination.
So, I took antibiotics before bed, meaning I took them at lunch and before sleep on 3/21, and I have been taking them until now, 3/22 evening.
Currently, I am still on antibiotics, but I occasionally feel uncomfortable, mainly with symptoms of bladder fullness and frequent urination.
Unlike the previous episodes where I experienced intense burning at the urethral opening, over the past three days (3/20 to 3/22), I have also experienced occasional dizziness and chills.
I'm unsure if this is related to the urinary issue.
I would appreciate your advice.
I have seen a doctor and had a urinalysis, which was normal, but I still feel uncomfortable, accompanied by slight dizziness and chills.
If further examinations are needed, what should I check for? Thank you, doctor.

Prostatitis should also be considered.
It is recommended to see a urologist for a thorough evaluation, including blood tests.

It sounds like you are experiencing a complex and frustrating situation with persistent urinary discomfort following a urinary tract infection (UTI). Your symptoms, including bladder pressure, frequent urination, and occasional dizziness and chills, can be distressing, especially after undergoing treatment with antibiotics. Here’s a comprehensive overview of what you might consider and discuss with your healthcare provider.

Understanding Your Symptoms
1. Persistent Discomfort: After a UTI, it is not uncommon for some individuals to experience lingering symptoms, even after the infection has been treated. This can be due to several factors, including:
- Irritation of the Urinary Tract: The urinary tract may remain sensitive or inflamed after the infection has cleared, leading to discomfort.

- Bladder Overactivity: Sometimes, the bladder can become overactive following an infection, causing symptoms like frequent urination and urgency.

- Incomplete Treatment: If the initial infection was not fully resolved, or if there is a new infection, symptoms may persist.

2. Dizziness and Chills: These symptoms can be concerning. While they may not be directly related to your urinary symptoms, they could indicate:
- Dehydration: If you are not drinking enough fluids, especially while on antibiotics, you may experience dizziness.

- Side Effects of Medication: Some antibiotics can cause side effects, including dizziness or gastrointestinal upset.

- Underlying Infection: If there is an ongoing infection, systemic symptoms like chills may occur.

Recommendations for Management
1. Follow-Up with Your Doctor: Since your urine tests came back normal, it’s essential to keep your healthcare provider informed about your ongoing symptoms. They may consider:
- Further Testing: If symptoms persist, additional tests such as a urine culture, imaging studies (like an ultrasound), or cystoscopy (a procedure to look inside the bladder) may be warranted to rule out other conditions.

- Referral to a Specialist: If your primary care physician or urologist cannot identify the issue, a referral to a specialist in pelvic floor disorders or a nephrologist may be beneficial.

2. Hydration: Ensure you are drinking plenty of fluids. Staying well-hydrated can help flush out any residual bacteria and soothe the urinary tract.

3. Dietary Considerations: Avoid irritants such as caffeine, alcohol, spicy foods, and artificial sweeteners, which can exacerbate bladder irritation. Incorporating foods rich in probiotics, like yogurt, may also help maintain a healthy urinary tract.

4. Symptom Management: Over-the-counter medications such as phenazopyridine can help relieve urinary discomfort, but they should not replace medical treatment. Discuss with your doctor if this is appropriate for your situation.

5. Monitor Symptoms: Keep a diary of your symptoms, noting when they occur and their severity. This information can be invaluable for your healthcare provider in determining the next steps.

6. Rest and Stress Management: Given that you were preparing for exams, stress could also play a role in your symptoms. Ensure you are getting adequate rest and consider stress-reduction techniques such as mindfulness or gentle exercise.

Conclusion
Persistent urinary discomfort after a UTI can be multifactorial, and it’s crucial to work closely with your healthcare provider to identify the underlying cause. While your urine tests are normal, ongoing symptoms warrant further investigation. Stay hydrated, maintain a healthy diet, and keep track of your symptoms to provide your doctor with the best information possible. If your symptoms worsen or new symptoms arise, seek medical attention promptly. Your health and comfort are paramount, and addressing these issues early can lead to better outcomes.
