Accuracy of Pregnancy Tests After Abortion
Hello, I underwent a surgical abortion on February 21 due to a blighted ovum at eight weeks.
Three days post-surgery, I had a follow-up ultrasound, and the doctor confirmed that everything was clear.
My husband and I are not actively avoiding pregnancy.
My menstrual cycle is approximately 33-35 days, and I consider the date of the surgery as the first day of my last period.
Therefore, I expect my next period around March 25.
Recently, I have been experiencing some bloating and fatigue.
This morning, March 23, I took a pregnancy test using my first morning urine.
Initially, within the first five minutes, there were very faint lines, but after about an hour, the lines became quite prominent.
Could this indicate that I am pregnant again, or could it be that the hCG from my previous pregnancy has not yet decreased? Thank you!
Vivi, 30~39 year old female. Ask Date: 2016/03/23

You may consider visiting a hospital to discuss with a physician and undergo examinations.
If necessary, you can also have a blood test for chorionic membrane index at a larger hospital for more accurate results.
Sincerely, Dr.

After undergoing an abortion due to a non-viable pregnancy at eight weeks, it's understandable to have concerns about the accuracy of pregnancy tests and the possibility of a new pregnancy. In your case, you had the abortion on February 21, and you are using that date as the start of your menstrual cycle, which is a common practice. Given your menstrual cycle length of approximately 33-35 days, you anticipated your next period around March 25.
When you took a pregnancy test on March 23, you observed a faint line that became more pronounced after an hour. This situation raises a few important points regarding the interpretation of pregnancy tests after an abortion.
1. Understanding hCG Levels: After an abortion, it can take time for the human chorionic gonadotropin (hCG) levels to return to zero. Depending on how high the hCG levels were before the abortion, it may take several days to weeks for them to drop completely. If the test you took is showing a faint line, it could indicate that residual hCG from the previous pregnancy is still present in your system.
2. Timing of the Test: The timing of your test is also crucial. Testing too early can lead to ambiguous results. Since you are using the date of your abortion as a reference point for your menstrual cycle, it is important to note that ovulation may occur approximately two weeks after the abortion, depending on your individual cycle and recovery. If you ovulated shortly after the abortion, it is possible to conceive again before your next expected period.
3. Interpreting the Test Results: A faint line on a pregnancy test can indicate a positive result, especially if it becomes more pronounced over time. However, the presence of a faint line could also suggest that hCG levels are still declining from the previous pregnancy. To clarify the situation, it is advisable to take another test in a few days. If you are indeed pregnant, the line should become darker as hCG levels rise.
4. Follow-Up with Healthcare Provider: Given your recent history of abortion and the current uncertainty regarding your pregnancy status, it is wise to follow up with your healthcare provider. They may recommend a blood test to measure your hCG levels quantitatively, which can provide a clearer picture of whether you are pregnant again or if the previous pregnancy's hCG levels are still affecting the test results.
5. Monitoring Symptoms: In addition to testing, pay attention to any symptoms you may experience, such as changes in bleeding, cramping, or other signs of early pregnancy. If you notice any unusual symptoms or have concerns, do not hesitate to reach out to your healthcare provider for guidance.
In summary, while it is possible that you could be pregnant again, the faint line on your pregnancy test could also be due to residual hCG from your previous pregnancy. To get a definitive answer, consider retesting in a few days and consult with your healthcare provider for further evaluation. Your health and peace of mind are paramount during this time, so seeking professional advice is always a prudent step.
