Pregnancy test questions?

Hello doctor, I would like to ask about my girlfriend.
After our last sexual intercourse, she used a pregnancy test on the 24th, 66th, and 78th days, and all results were negative (there was no sexual intercourse in between, and the test on the 78th day was done using morning urine).
During that period, she also had her menstrual period twice, with normal flow lasting 7 days each time.
However, this month her period is already 6 days late.
Doctor, after such a long time, is it still possible for the pregnancy test to show a negative result, or is it just a case of irregular menstruation? Thank you for your response!

Xiao Ke, 10~19 year old female. Ask Date: 2016/03/23

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, a pregnancy test should be taken at least 12 days after conception to get accurate results.
The chances of pregnancy are low; it is likely just a menstrual irregularity.
Wishing you good health.

When it comes to understanding pregnancy tests and irregular menstrual cycles, there are several factors to consider. Based on your description, your girlfriend has taken multiple pregnancy tests over a span of time, all of which returned negative results, even after a significant period following the last sexual encounter. Additionally, she has experienced two normal menstrual cycles during this time, which further complicates the situation.

Firstly, it is important to understand how pregnancy tests work. Most home pregnancy tests detect the hormone human chorionic gonadotropin (hCG), which is produced shortly after a fertilized egg attaches to the uterine lining. Typically, hCG can be detected in urine about 14 days after conception. If your girlfriend had negative results at 24 days, 66 days, and 78 days after the last sexual encounter, it is highly unlikely that she is pregnant, especially since she had normal menstrual periods during that time.

The fact that she experienced two menstrual cycles with normal flow (lasting about 7 days each) suggests that her body is functioning normally in terms of ovulation and menstruation. Menstrual cycles can sometimes be irregular due to various factors such as stress, changes in weight, hormonal imbalances, or underlying health conditions. Given that her period is now late by 6 days, it could be due to one of these factors rather than pregnancy.

Regarding your question about whether a pregnancy test could still yield a negative result after such a long time, it is highly improbable. By this point, if she were pregnant, the hCG levels would be high enough to be detected by a home pregnancy test. The tests are generally very sensitive and can detect even low levels of hCG. If she has had three negative tests and two normal periods, it is safe to conclude that she is not pregnant.

However, if her period continues to be delayed or if she experiences other symptoms such as severe pain, unusual discharge, or any other concerning signs, it would be advisable for her to consult a healthcare provider. A doctor can perform a more sensitive blood test for hCG and evaluate her overall reproductive health.
In summary, based on the information provided, it seems unlikely that your girlfriend is pregnant given the negative test results and the occurrence of normal menstrual cycles. The delay in her current period could be attributed to other factors, and if it persists, seeking medical advice would be the best course of action.
