Is it possible to take a pregnancy test?
Hello Doctor: I got married in January of this year, and we have not used contraception since then.
After trying for two months without success, I started seeing a Traditional Chinese Medicine (TCM) practitioner in March to improve my health and began tracking my basal body temperature (BBT).
I would like to ask if my BBT chart looks normal.
On March 3rd, the second day of my menstrual period, I started TCM treatment.
The practitioner mentioned that my body constitution is relatively cold and prescribed a seven-day course of medicine, advising me to start measuring my BBT.
Here are my BBT readings:
- 3/4: 36.41°C
- 3/5: 35.84°C
- 3/6: 36.06°C
- 3/7: 36.15°C
- 3/8: 36.19°C (end of menstruation; my period was from 3/2 to 3/8)
- 3/9: 35.99°C
- 3/10: 35.99°C (returned for TCM follow-up, received another seven-day prescription)
- 3/11: 35.59°C (intercourse; recorded lowest temperature)
- 3/12: 36.18°C
- 3/13: 36.43°C (intercourse)
- 3/14: 36.69°C (intercourse; this temperature marks the beginning of the high temperature phase)
- 3/15: 36.68°C
- 3/16: 36.50°C (intercourse)
- 3/17: 36.43°C
- 3/18: 36.61°C (intercourse)
- 3/19: 36.46°C (went to bed later than usual, around 1:00 AM)
- 3/20: 36.46°C (stayed up until 2:30 AM the previous night)
- 3/21: 36.55°C
- 3/22: 36.77°C
- 3/23: 36.87°C
- 3/24: 36.98°C (highest temperature recorded so far; three consecutive days of peak temperatures)
Currently, I have been in the high temperature phase for 11 days, with the last three days showing even higher temperatures.
Based on my temperature records, is there a possibility that implantation has occurred? Should I take a pregnancy test tomorrow, or is there a low likelihood of pregnancy? Do you have any recommendations on when to take the pregnancy test? Thank you, Doctor.

Dr. Lü Lizheng reply Obstetrics and Gynecology
The basal body temperature can only provide a reference for ovulation and cannot assess whether pregnancy has occurred.
If menstruation does not arrive 18 days after intercourse, it is recommended to take a pregnancy test for an accurate evaluation of pregnancy.

More Info
Understanding your basal body temperature (BBT) can provide valuable insights into your menstrual cycle and potential pregnancy. BBT is the body's temperature at rest, and it typically rises slightly after ovulation due to increased progesterone levels. Monitoring BBT can help identify ovulation and the luteal phase, which is crucial for those trying to conceive.
From your records, it appears that you have been tracking your BBT diligently since the end of your menstrual cycle. The pattern you've observed shows a significant rise in temperature after March 14, which is consistent with the post-ovulation phase. The sustained higher temperatures (above 36.5°C) for over 11 days could indicate that you have ovulated and that your body is maintaining a higher temperature due to the presence of progesterone.
In terms of pregnancy, a sustained rise in BBT for more than 14 days is often a sign that implantation may have occurred, especially if your temperature remains elevated. However, it's important to note that BBT alone is not a definitive indicator of pregnancy. Other factors, such as stress, illness, or changes in sleep patterns, can also affect your temperature readings.
Regarding your question about whether you should take a pregnancy test, it is generally recommended to wait until at least the first day of your missed period for the most accurate results. However, if you are experiencing symptoms of pregnancy (such as nausea, breast tenderness, or fatigue), you might consider testing a few days earlier. Home pregnancy tests are quite sensitive and can often detect pregnancy hormones (hCG) as early as a few days before your expected period.
If your BBT continues to remain elevated and you receive a positive pregnancy test, it would be a good idea to schedule an appointment with your healthcare provider. They can perform a blood test to confirm the pregnancy and check for any potential issues, especially since you mentioned that you have been working with a traditional Chinese medicine practitioner to improve your health.
It's also worth noting that fluctuations in BBT can occur, and while a drop in temperature can sometimes indicate the onset of menstruation, it does not always mean that pregnancy has not occurred. If your temperature drops but you still suspect you might be pregnant, consider taking another test a few days later.
In summary, your BBT readings suggest that you may have ovulated and that there is a possibility of pregnancy. While it is tempting to test immediately, waiting until your expected period can provide more reliable results. If you have any concerns or experience unusual symptoms, do not hesitate to reach out to your healthcare provider for personalized advice and support.
