Glans and foreskin issues?

Hello doctor, for the past few weeks, the glans and foreskin have been ulcerated and painful.
It’s difficult to retract the foreskin during bathing or urination; even a slight pull causes significant pain.
However, I still endure the discomfort to clean the wounds.
I remembered that although it wasn’t as severe before, I had redness and ulceration, and I used a povidone-iodine ointment recommended by another doctor, which worked fairly well at that time.
Last night after my shower, I applied it, but the ointment caused intense pain when it touched the wounds.
This morning, I noticed there was no discharge resembling pus or ointment, but the redness and swelling seem to be about the same.
Do you think I should give the povidone-iodine another try, or should I continue using the current medication, Linidron? What can I do to promote faster healing? Thank you, doctor!

More Info

It sounds like you are experiencing some significant discomfort related to your glans and foreskin, which can be quite distressing. Based on your description, it seems you may be dealing with a condition known as balanitis, which is inflammation of the glans (the head of the penis) and can sometimes involve the foreskin as well. This condition can be caused by various factors, including infections (bacterial, fungal, or yeast), irritants (such as soaps or lotions), or even skin conditions like eczema or psoriasis.

Understanding Your Condition
1. Symptoms: The symptoms you describe—pain, redness, swelling, and difficulty retracting the foreskin—are common in cases of balanitis. The fact that you have experienced this before suggests that it may be a recurring issue.

2. Potential Causes:
- Infections: Bacterial infections, such as those caused by Staphylococcus or Streptococcus, can lead to balanitis. Fungal infections, particularly Candida (yeast), are also common.

- Irritation: Products like soaps, lotions, or even certain fabrics can irritate the sensitive skin of the glans and foreskin.

- Poor Hygiene: Inadequate cleaning can lead to the accumulation of smegma, which can irritate the area.

- Skin Conditions: Conditions like eczema or psoriasis can also affect the genital area.

Treatment Options
1. Topical Treatments:
- Antiseptic Creams: While you mentioned using a povidone-iodine ointment (often referred to as Betadine), it can be irritating for some individuals, especially on sensitive skin. If it causes pain, it may be best to discontinue its use.

- Hydrocortisone Cream: A mild topical steroid can help reduce inflammation and itching. However, it should be used cautiously and not for prolonged periods without medical supervision.

- Antifungal Creams: If a yeast infection is suspected, an antifungal cream (like clotrimazole or miconazole) may be beneficial.

2. Hygiene Practices:
- Gentle Cleaning: Clean the area gently with warm water and mild soap. Avoid harsh soaps or fragrances that can irritate the skin.

- Drying: Ensure the area is thoroughly dried after washing, as moisture can exacerbate fungal infections.

3. Avoid Irritants:
- Avoid using any products that may irritate the area, including scented soaps, lotions, or other topical agents until the condition improves.

4. Consultation with a Healthcare Provider:
- Given the severity of your symptoms and the lack of improvement, it is crucial to consult a healthcare provider, preferably a urologist or dermatologist. They can perform a physical examination and may recommend further tests to determine the exact cause of your symptoms.

5. Follow-Up Care:
- If you are prescribed medication or a specific treatment plan, be sure to follow up with your healthcare provider if symptoms do not improve within a few days or worsen.

Conclusion
In summary, while topical treatments can be effective, the key to resolving your symptoms lies in identifying the underlying cause. Given your experience with povidone-iodine and the pain it caused, it may be wise to explore other options, such as antifungal or mild steroid creams, under the guidance of a healthcare professional. Proper hygiene and avoiding irritants are also essential in managing your condition. Please seek medical attention promptly to ensure you receive the appropriate care and treatment.
