Cracking on the surface of the tongue?
Recently, my mother's tongue has developed severe cracks.
It started with just a few minor fissures, but after visiting a doctor, they said it was nothing serious.
They prescribed an ointment, but it hasn't improved.
Recently, the cracks have returned, and they are larger and more pronounced than they were 2 to 3 months ago.
Sometimes her tongue hurts, and other times it doesn't.
She also feels a heaviness in her head.
Could a professional physician explain what this condition might be? What should we be aware of? Do you recommend any specific doctors or hospitals in Taipei? Thank you for your response! We appreciate it!

Dr. Zeng Guanghui reply Dentistry
It may be caused by an abnormal immune system, high emotional stress, poor sleep quality, or excessive dryness in the mouth.
It is recommended to consult an oral surgeon at Taipei Medical Center for further examination.

Understanding tongue cracks, also known as fissured tongue, can be quite complex, as it may stem from various underlying causes. Fissured tongue is characterized by grooves or cracks on the surface of the tongue, which can vary in depth and size. While it is often benign, it can sometimes be associated with other health conditions.
Causes of Fissured Tongue
1. Genetic Factors: Some individuals may have a hereditary predisposition to developing fissured tongue. If family members have similar conditions, it may be a genetic trait.
2. Nutritional Deficiencies: Deficiencies in certain vitamins and minerals, particularly B vitamins (like B2, B3, and B12), iron, and folic acid, can lead to changes in the tongue's appearance, including fissures.
3. Dehydration: Insufficient hydration can cause the tissues of the tongue to dry out, leading to cracks.
4. Systemic Conditions: Conditions such as psoriasis, Sjögren's syndrome, or geographic tongue can manifest as fissured tongue. These conditions may require further medical evaluation.
5. Oral Hygiene: Poor oral hygiene can contribute to the development of fissured tongue, as bacteria can accumulate in the grooves, leading to inflammation and discomfort.
Symptoms
- Visible Cracks or Grooves: The most noticeable symptom is the presence of cracks or grooves on the tongue's surface.
- Pain or Discomfort: Some individuals may experience pain or discomfort, especially if food particles get trapped in the fissures or if the tongue becomes irritated.
- Heavy Sensation: A feeling of heaviness or discomfort in the tongue may occur, particularly if the fissures are deep or inflamed.
Treatment Options
1. Maintain Oral Hygiene: Regular brushing of the tongue with a soft toothbrush can help remove debris and bacteria from the fissures. Using a tongue scraper may also be beneficial.
2. Hydration: Ensure adequate fluid intake to keep the tongue moist and prevent further cracking.
3. Nutritional Support: If deficiencies are suspected, consider dietary adjustments or supplements after consulting with a healthcare provider.
4. Topical Treatments: Over-the-counter topical ointments or prescribed medications may help alleviate discomfort. If a specific condition is diagnosed, targeted treatments may be necessary.
5. Regular Check-ups: If the condition persists or worsens, follow up with a healthcare provider or a specialist, such as a dentist or an oral surgeon, for further evaluation.
When to Seek Further Help
If your mother's condition continues to worsen despite treatment, or if she experiences additional symptoms such as swelling, persistent pain, or changes in taste, it is crucial to seek further medical advice. A referral to an oral medicine specialist or a dermatologist may be warranted for a more thorough investigation.
Recommended Hospitals in Taipei
In Taipei, several reputable hospitals and clinics specialize in oral health and can provide comprehensive care for tongue-related issues. Some of the well-regarded institutions include:

- Taipei Medical University Hospital: Offers specialized dental services and has a team of experienced professionals.
- Chang Gung Memorial Hospital: Renowned for its advanced medical care and research, including oral health specialties.
In conclusion, fissured tongue can be a benign condition, but it may also indicate underlying health issues. Maintaining good oral hygiene, staying hydrated, and addressing any nutritional deficiencies are essential steps in managing the condition. If symptoms persist or worsen, seeking specialized care is crucial for proper diagnosis and treatment.
