The perineum is red, swollen, and very itchy?

I have redness and itching in my external vulva, and there is also some redness in the internal vulva.
The itchy area is located just above the clitoris.
I have previously used suppositories and applied antibiotics, which provided some improvement, but the itching returned after a few days.
Occasionally, I also notice a whitish discharge.

Itchy and swollen perineum can be a distressing condition, and understanding its causes and potential solutions is essential for effective management. The perineum, which is the area between the anus and the genitals, can become irritated for various reasons, leading to symptoms such as itching, swelling, and discharge.

Common Causes
1. Infections:
- Yeast Infections: One of the most common causes of itching and irritation in the genital area is a yeast infection, often caused by Candida species. Symptoms may include intense itching, redness, and a thick, white discharge.

- Bacterial Infections: Conditions such as bacterial vaginosis can also lead to irritation and unusual discharge. This condition is characterized by a fishy odor and may cause discomfort.

- Sexually Transmitted Infections (STIs): Infections like herpes, gonorrhea, or chlamydia can cause itching, swelling, and discharge. If you suspect an STI, it is crucial to seek medical attention.

2. Skin Conditions:
- Contact Dermatitis: This occurs when the skin reacts to irritants or allergens, such as soaps, detergents, or personal hygiene products. Symptoms include redness, itching, and swelling.

- Psoriasis or Eczema: These chronic skin conditions can also affect the genital area, leading to itching and discomfort.

3. Hygiene Practices:
- Poor hygiene can lead to the accumulation of sweat and bacteria, causing irritation. Conversely, excessive washing or using harsh soaps can strip the skin of its natural oils, leading to dryness and irritation.

4. Hormonal Changes:
- Hormonal fluctuations, particularly during menstruation or menopause, can lead to changes in the vaginal flora and skin sensitivity, resulting in itching and discomfort.

5. Allergic Reactions:
- Allergies to certain materials, such as latex in condoms or certain fabrics, can cause localized irritation and itching.

Solutions and Management
1. Maintain Good Hygiene:
- Gently wash the area with mild soap and water. Avoid using scented products that can irritate the skin. Pat the area dry instead of rubbing it.

2. Topical Treatments:
- Over-the-counter antifungal creams can be effective for yeast infections. If a bacterial infection is suspected, a healthcare provider may prescribe antibiotics.

- Hydrocortisone cream may help reduce inflammation and itching, but it should be used sparingly and under the guidance of a healthcare provider.

3. Avoid Irritants:
- Identify and avoid potential irritants, such as certain soaps, detergents, or fabrics. Opt for breathable cotton underwear and avoid tight-fitting clothing.

4. Dietary Considerations:
- A diet low in sugar may help reduce the frequency of yeast infections. Probiotics can also support healthy vaginal flora.

5. Consult a Healthcare Provider:
- If symptoms persist despite home treatments, or if you notice unusual discharge or other concerning symptoms, it is essential to consult a healthcare provider. They may conduct tests to determine the underlying cause and recommend appropriate treatment.

6. Consider Hormonal Factors:
- If hormonal changes are suspected to be a contributing factor, discussing this with a healthcare provider may lead to solutions such as hormone therapy or other interventions.

Conclusion
Itchy and swollen perineum can arise from various causes, including infections, skin conditions, hygiene practices, and hormonal changes. Identifying the underlying cause is crucial for effective treatment. If you have tried over-the-counter treatments and have not seen improvement, or if you experience additional symptoms, it is important to seek medical advice for a thorough evaluation and tailored treatment plan. Remember that maintaining good hygiene, avoiding irritants, and consulting with a healthcare professional are key steps in managing this condition effectively.
