Pimples located beneath the penis?

Hello doctor, I recently had a circumcision and noticed a large bump under the glans that resembles an acne pimple.
It has a yellowish-white pus inside, and it hurts when I have an erection, but not when I'm flaccid.
There are also a few pearly penile papules nearby.
I'm wondering if this is harmful to my health.

Dr. Li Xinyi reply Urology

It is recommended to see a urologist for examination and medication treatment.

More Info

When it comes to the appearance of pimples or lesions below the penis, particularly after a circumcision, there are several potential causes and considerations to keep in mind. The description you provided suggests that you may be experiencing a condition known as folliculitis, which is an inflammation of the hair follicles. This can occur due to a variety of reasons, including irritation, infection, or blockage of the follicles.

Possible Causes
1. Folliculitis: This is often characterized by red, inflamed bumps that may contain pus. It can be caused by bacterial infections, irritation from shaving or friction, or even fungal infections. Given that you mentioned the lesion is painful when erect, this could indicate inflammation or infection of the hair follicle.

2. Pearly Penile Papules: The "pearl-like" bumps you mentioned could be pearly penile papules, which are harmless and common in males. They are typically small, dome-shaped, and occur around the corona of the glans (the head of the penis). They are not associated with any disease and do not require treatment.

3. Sebaceous Cysts: These are small lumps that can form under the skin and may contain a thick, yellowish substance. They are generally harmless and can occur anywhere on the body, including the genital area.

4. Genital Warts: Caused by the human papillomavirus (HPV), these can appear as small, flesh-colored or gray growths. They are typically painless, but they can cause discomfort depending on their location.

5. Herpes Simplex Virus: If the lesion is painful and has a blister-like appearance, it could be a sign of a herpes infection. This would typically be accompanied by other symptoms, such as itching or burning.

Concerns and Recommendations
- Pain and Discomfort: Since you mentioned that the lesion is painful when erect, it is essential to monitor this symptom. Pain can indicate an infection or other underlying issue that may require medical attention.

- Consult a Healthcare Provider: It is advisable to consult a healthcare provider, preferably a urologist or dermatologist, for an accurate diagnosis. They may perform a physical examination and, if necessary, order tests to determine the exact nature of the lesion.

- Avoid Self-Diagnosis: While it can be tempting to self-diagnose based on symptoms, many conditions can present similarly. A healthcare professional can provide clarity and appropriate treatment options.

- Hygiene: Maintaining good hygiene is crucial, especially in the genital area. Ensure that the area is clean and dry, and avoid using harsh soaps or products that may irritate the skin.

- Avoid Picking or Squeezing: It is important not to pick at or squeeze the lesion, as this can lead to further irritation or infection.

Conclusion
In summary, while the appearance of a pimple-like lesion below the penis can be concerning, many potential causes range from benign to more serious conditions. Given the pain associated with the lesion, it is essential to seek medical advice to ensure proper diagnosis and treatment. Early intervention can help prevent complications and provide peace of mind regarding your health.
