Menstruation is delayed or absent?

Hello doctor, my period was very late in March and I've been feeling a heaviness in my abdomen.
Today I noticed a little bit of blood, but when I used a sanitary pad, it didn't absorb much blood and was quite clean.
However, I still feel a heaviness in my abdomen.
My periods have always been very regular, and this is the first time I've experienced this issue of my period being delayed.
I haven't had any sexual intercourse.
What could be the reason for this?

Xiao An, 20~29 year old female. Ask Date: 2016/03/28

Hello, the factors that can affect the menstrual cycle or cause intermenstrual bleeding are numerous, including environmental factors, lifestyle, stress, emotional instability, diet, age, and lesions in the uterus or cervix, among others.
It is advisable to consult a gynecologist for examination and treatment.
Thank you! Wishing you peace and health, Dr.
Chang.

Irregular periods can be a source of concern for many women, especially when they experience changes in their menstrual cycle that deviate from what they consider normal. In your case, it appears that you are experiencing a delay in your menstrual cycle, accompanied by some abdominal discomfort and minimal spotting. Let's explore the potential causes and concerns related to irregular periods.

First, it's important to understand what constitutes a normal menstrual cycle. A typical cycle ranges from 21 to 35 days, with menstruation lasting anywhere from 2 to 7 days. Variations in cycle length and flow can occur due to various factors, including hormonal fluctuations, stress, lifestyle changes, and underlying health conditions.

In your situation, since you mentioned that your periods were previously regular, the delay and the accompanying symptoms could be attributed to several factors:
1. Hormonal Imbalances: Hormones play a crucial role in regulating the menstrual cycle. Any disruption in the balance of hormones such as estrogen and progesterone can lead to irregularities. This can happen due to stress, significant weight changes, or changes in physical activity levels.

2. Stress and Lifestyle Changes: Psychological stress can significantly impact your menstrual cycle. If you've been experiencing increased stress or changes in your daily routine, this could be a contributing factor. Additionally, lifestyle changes, such as changes in diet, exercise, or sleep patterns, can also affect your cycle.

3. Ovulation Issues: Sometimes, the body may not ovulate as expected, leading to anovulatory cycles. This means that while you may experience some bleeding, it may not be a true menstrual period. Instead, it could be spotting due to hormonal fluctuations.

4. Underlying Health Conditions: Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or other hormonal disorders can lead to irregular periods. While you mentioned that you have not engaged in sexual activity, it's still important to consider these factors.

5. Age and Developmental Factors: If you are in your late teens or early twenties, your body may still be adjusting to hormonal changes, which can lead to irregular cycles. This is particularly common in the first few years after menarche (the first menstrual period).

Given your symptoms of abdominal discomfort and minimal spotting, it is advisable to monitor your situation closely. If your period does not arrive within a week or if you experience severe pain or other concerning symptoms, it would be wise to consult a healthcare provider. They may recommend a pelvic exam, blood tests to check hormone levels, or an ultrasound to rule out any underlying issues.

In conclusion, while irregular periods can be concerning, they are often a normal part of a woman's reproductive health. Factors such as stress, hormonal imbalances, and lifestyle changes can all contribute to these irregularities. If you continue to experience irregularities or have additional concerns, seeking medical advice is the best course of action to ensure your health and well-being.
