Menstruation has not occurred?

Hello doctor, I would like to ask about my last sexual encounter with my girlfriend on March 11.
It was brief; there was only penetration for a moment, and I withdrew without ejaculation.
Since then, we have not had any sexual activity.
My girlfriend was supposed to get her period on March 25 (28-day cycle), but it has not arrived yet.
On March 29, I bought a pregnancy test and the result showed one line.
Is there still a possibility of pregnancy? What should we do next?

Ccc, 30~39 year old female. Ask Date: 2016/03/30

Dr. Lü Lizheng reply Obstetrics and Gynecology

Regarding the pregnancy test results, it has been 18 days since the sexual intercourse, and the results can be considered reliable, indicating that pregnancy is unlikely.
It may be due to irregular menstruation, and a medical examination and treatment should be sought.
Sincerely, Dr.

When it comes to missed periods and the possibility of pregnancy, it's essential to understand the various factors at play. In your case, since you had sexual intercourse on March 11 and your girlfriend's expected period was on March 25, the timing is crucial. The menstrual cycle can vary significantly among individuals, and even a slight change in stress levels, diet, or health can cause a delay in menstruation.

Given that your girlfriend's period is late, and she tested negative for pregnancy on March 29, there are a few possibilities to consider. First, it is important to note that home pregnancy tests are generally reliable, especially when taken after a missed period. However, they can sometimes yield false negatives, particularly if taken too early. If ovulation occurred later than expected, it might not have been enough time for the hormone hCG (human chorionic gonadotropin) to build up to detectable levels in her urine.

Here are some steps you can take moving forward:
1. Wait and Retest: Since it has been a few days since the initial test, it may be beneficial for your girlfriend to wait a few more days and retest. This will allow more time for hCG levels to rise if she is indeed pregnant.

2. Consider Other Factors: If the second test is also negative, it might be worth considering other reasons for the missed period. Stress, changes in weight, illness, or hormonal imbalances can all contribute to menstrual irregularities.
3. Consult a Healthcare Provider: If her period does not arrive within a week or two after the missed date, or if she continues to experience symptoms such as nausea, breast tenderness, or fatigue, it would be wise to consult a healthcare provider. They can perform a blood test, which is more sensitive than urine tests, and can also evaluate for other potential causes of the missed period.

4. Monitor Symptoms: Keep an eye on any additional symptoms that may arise. If she experiences severe abdominal pain, heavy bleeding, or any other concerning symptoms, seek medical attention immediately.

5. Discuss Contraception: If pregnancy is not desired, consider discussing contraceptive options with your girlfriend. Understanding and utilizing effective birth control methods can help prevent unintended pregnancies in the future.

In summary, while the chances of pregnancy seem low given the negative test result and the nature of the sexual encounter, it is not impossible. The best course of action is to wait a few days and retest, and if necessary, consult a healthcare professional for further evaluation. It’s essential to approach this situation with care and open communication with your girlfriend to ensure both of you feel supported during this time.
