Coughing for nearly a month and a half, the clinic doctor said it is not sinusitis?

Hello, doctor.
I have been coughing for nearly a month and a half.
I wear a mask while at work, but I still experience nasal congestion, swelling, and pain while sleeping; I even have some bleeding and nighttime coughing.
The next morning, I often have a lot of yellow-green nasal discharge and phlegm.
Today, I also started experiencing pain on the left side of my throat, and when I cough, I tear up, have nasal discharge, and feel nauseous.
I would like to ask which specialty I should consult? What tests might be necessary? Can a general clinic address this issue?

Zhang Yu, 20~29 year old female. Ask Date: 2016/03/30

Dr. Huang Yiwen reply Pulmonology

More Info

Based on your symptoms and the duration of your persistent cough, it is advisable to consult with a specialist who can provide a more thorough evaluation and appropriate treatment. Given that you have been experiencing a cough for over a month, along with nasal congestion, pain, and the presence of yellow-green mucus, it is likely that you may be dealing with a chronic respiratory condition, possibly related to sinusitis or bronchitis.

Recommended Specialist
1. Pulmonologist: This is a specialist in respiratory diseases. Since you have a persistent cough and respiratory symptoms, a pulmonologist can perform a detailed assessment of your lung function and rule out conditions such as chronic bronchitis, asthma, or other pulmonary disorders.

2. Otolaryngologist (ENT Specialist): Given your nasal symptoms, including swelling, pain, and the presence of nasal discharge, an ENT specialist would be beneficial. They can evaluate for conditions such as sinusitis, nasal polyps, or other issues affecting the upper respiratory tract.

Possible Tests and Examinations
1. Chest X-ray: This imaging test can help rule out pneumonia, lung infections, or other structural abnormalities in the lungs.

2. CT Scan of the Sinuses: If sinusitis is suspected, a CT scan can provide detailed images of your sinuses and help identify any blockages or infections.

3. Pulmonary Function Tests: These tests measure how well your lungs are working and can help diagnose conditions like asthma or chronic obstructive pulmonary disease (COPD).

4. Nasal Endoscopy: This procedure allows the doctor to look inside your nasal passages and sinuses using a thin, flexible tube with a camera, which can help identify any abnormalities.

5. Allergy Testing: If allergies are suspected to be contributing to your symptoms, allergy testing may be recommended to identify specific triggers.

General Practitioner vs. Specialist
While a general practitioner (GP) can provide initial care and may be able to manage mild cases of respiratory infections, your symptoms suggest a more complex issue that may require specialized care. A GP can perform a preliminary evaluation and may initiate treatment, but if your symptoms persist or worsen, they will likely refer you to a specialist for further investigation.

Additional Considerations
- Hydration and Humidity: Ensure you are drinking plenty of fluids and consider using a humidifier at home to help alleviate nasal congestion and throat irritation.

- Avoid Irritants: Stay away from smoke, strong odors, and allergens that may exacerbate your symptoms.

- Rest: Adequate rest is crucial for recovery, especially if your body is fighting an infection.

- Follow-Up: If you notice any worsening of symptoms, such as increased difficulty breathing, high fever, or significant changes in your cough, seek medical attention promptly.

In summary, given the complexity and duration of your symptoms, it is best to see a pulmonologist or an ENT specialist for a comprehensive evaluation. They will be able to conduct the necessary tests and provide a tailored treatment plan to address your persistent cough and associated symptoms effectively.
