Abdominal pain after intercourse?

Hello doctor, I had intercourse with my boyfriend the day after my menstrual period ended.
About four hours later, I experienced abdominal pain in the area of my uterus.
I have felt some discomfort in previous instances, but it wasn't as prolonged as this time.
I have polycystic ovary syndrome (PCOS), and I also experienced stomach pain and diarrhea for two days during the first five days of my period.
Could you please help diagnose what might be happening? Thank you.

If abdominal pain persists for more than 1-2 days after sexual intercourse, it is advisable to seek medical evaluation.

Post-sex abdominal pain can be a concerning symptom, especially when it occurs in conjunction with other gastrointestinal issues like stomach pain and diarrhea. Given your history of polycystic ovary syndrome (PCOS) and the timing of your symptoms, there are several potential causes to consider.

Firstly, the pain you are experiencing in the abdominal area after sexual intercourse could be attributed to several factors. One common cause is pelvic inflammatory disease (PID), which can occur if there is an infection in the reproductive organs. PID can lead to inflammation and pain in the lower abdomen, particularly after sexual activity. Symptoms may include fever, unusual discharge, and pain during intercourse, but it can also present with more subtle signs like the discomfort you are describing.

Another possibility is that the pain is related to your menstrual cycle. Since you mentioned that your period had just ended, it is possible that you are experiencing ovulation pain (also known as mittelschmerz), which can occur around the time of ovulation. This pain is typically localized to one side of the abdomen and can be sharp or cramp-like. However, given that you have PCOS, your ovulation may not follow a typical pattern, which could complicate this scenario.

Additionally, the discomfort could be linked to gastrointestinal issues. Since you experienced stomach pain and diarrhea prior to the abdominal pain, it is essential to consider whether there is a gastrointestinal infection or another digestive issue at play. Conditions such as irritable bowel syndrome (IBS) or even food intolerances can lead to abdominal pain that may be exacerbated by sexual activity due to the physical pressure on the abdomen.

Moreover, if you are experiencing discomfort in the uterus area, it could also be related to the presence of ovarian cysts, which are common in individuals with PCOS. These cysts can sometimes cause pain, especially if they rupture or if there is increased pressure during sexual activity.

It is also worth considering the possibility of a urinary tract infection (UTI), which can cause lower abdominal pain and discomfort during or after sex. Symptoms of a UTI may include a burning sensation during urination, frequent urges to urinate, and lower abdominal pain.

Given your symptoms and medical history, it is crucial to consult with a healthcare provider who can perform a thorough evaluation. This may include a pelvic exam, ultrasound, or other diagnostic tests to determine the underlying cause of your pain. It is essential to rule out any serious conditions, especially since you have a history of PCOS, which can complicate your reproductive health.

In summary, post-sex abdominal pain can arise from various causes, including pelvic inflammatory disease, ovulation pain, gastrointestinal issues, ovarian cysts, or urinary tract infections. Given the complexity of your symptoms and medical history, seeking medical advice is the best course of action to ensure proper diagnosis and treatment. Remember to provide your healthcare provider with as much detail as possible about your symptoms, including their timing, intensity, and any other associated symptoms, to aid in their assessment.
