Persistent Swelling and Discomfort in Your Right Eye - Ophthalmology

The right eye often feels swollen?

Hello Doctor: I would like to ask about my right eyelid.
It had swollen up on its own before and was a bit painful.
I initially thought it was a stye, but since it didn't swell much and resolved on its own after a few days, I didn't seek medical attention.
However, now I sometimes feel a small lump inside my eyelid, which is not painful, but my right eye often feels swollen, and sometimes it feels like I have no sensation when moving my eyeball.
This is most common right after waking up or after reading or using the computer for too long.
What could be the issue? I have been troubled by this for a while and hope to find a way to improve it.

A Zhao, 10~19 year old female. Ask Date: 2016/04/03

Dr. Wang Zhangji reply Ophthalmology

The sensation of eye swelling is commonly associated with ciliary muscle fatigue, which can be alleviated with eye drops or moderate rest.

More Info

Persistent swelling and discomfort in the eye can be concerning, and it’s important to understand the potential causes and appropriate actions to take. Based on your description, it seems that you have experienced swelling in your right eyelid, which initially seemed like a stye (or "針眼" in Chinese). Although the swelling subsided on its own, the presence of a small lump that you can feel inside the eyelid, along with recurrent swelling and discomfort, suggests that there may be an underlying issue that needs to be addressed.

Possible Causes
1. Chalazion: This is a common condition that occurs when a meibomian gland (which produces oil to lubricate the eye) becomes blocked. It can lead to a painless lump in the eyelid, which may cause swelling and discomfort. Unlike a stye, which is usually painful and caused by an infection, a chalazion is typically not painful and can persist for a longer time.

2. Stye (Hordeolum): A stye is an infection of the oil glands in the eyelid, leading to a painful, swollen bump. While you mentioned that it seemed to resolve on its own, it’s possible that a stye could have contributed to the initial swelling.

3. Allergic Reaction: Allergies can cause swelling and discomfort in the eyes, often accompanied by itching or redness. If you have been exposed to allergens (like pollen, dust, or pet dander), this could be a contributing factor.

4. Conjunctivitis: Also known as pink eye, this condition can cause redness, swelling, and discomfort in the eye. It can be caused by infections (viral or bacterial) or allergies.

5. Dry Eye Syndrome: Prolonged screen time or insufficient tear production can lead to dry eyes, which may cause discomfort, a feeling of swelling, and sometimes a sensation of a foreign body in the eye.

Recommendations
1. Consult an Eye Specialist: Given the persistent nature of your symptoms, it is advisable to see an ophthalmologist. They can perform a thorough examination of your eye and eyelid to determine the exact cause of your discomfort and swelling.

2. Warm Compresses: Applying a warm compress to the affected eyelid can help alleviate swelling and promote drainage if there is a blockage in the glands.

3. Avoid Rubbing Your Eyes: This can exacerbate irritation and lead to further complications.

4. Monitor Symptoms: Keep track of any additional symptoms that may arise, such as changes in vision, increased redness, or discharge from the eye. This information can be helpful for your healthcare provider.

5. Limit Screen Time: If you notice that your symptoms worsen after prolonged screen use, consider taking regular breaks using the 20-20-20 rule: every 20 minutes, look at something 20 feet away for at least 20 seconds.

6. Stay Hydrated: Ensure you are drinking enough water throughout the day, as hydration can help maintain eye moisture.

7. Consider Allergies: If you suspect that allergies may be contributing to your symptoms, consider discussing this with your doctor. They may recommend antihistamines or other treatments.

Conclusion
Persistent swelling and discomfort in the eye can stem from various causes, including blocked glands, infections, or allergic reactions. It is crucial to seek professional medical advice to receive an accurate diagnosis and appropriate treatment. Early intervention can help prevent complications and improve your overall eye health. Remember to take care of your eyes, and don’t hesitate to reach out to a healthcare professional for any concerns.
