Arrhythmia
Hello Dr.
He,
I would like to ask about my heart.
Sometimes it beats irregularly, and it feels like my heart is being jolted.
Occasionally, it will suddenly race for 20 to 40 beats in a row.
I also experience a sensation of pressure in my head, and each time it occurs, my face becomes flushed, and I feel a warmth in my body, followed by chills.
However, when I go to the hospital or the emergency room, they often cannot find any cause.
I have undergone a 24-hour Holter monitor, echocardiogram, exercise stress test, and vascular ultrasound, but nothing significant has been detected.
They only mentioned that I have 5 to 10 irregular heartbeats throughout the day.
However, during episodes, I feel very uncomfortable, and I also notice that some of my muscles frequently twitch.
I would like to know what type of arrhythmia this might be and how I can improve this issue, as it has made me hesitant to exercise and work.
Are there any other methods to identify the underlying problem for treatment? (Thank you very much, Dr.
He.)

Dr. He Dongjin reply Cardiology
Hello, it sounds like you are experiencing benign arrhythmia, more specifically symptoms following premature ventricular contractions.
Previous discomfort may have caused some autonomic nervous system dysregulation, leading to a vicious cycle of panic and anxiety symptoms.
You may consider taking some medications to help soothe the autonomic nervous system.
If you feel the need to investigate the arrhythmia further, you can visit a medical center's cardiology department to undergo a week-long rhythm monitoring.
Peace and care from Changhua Hospital.

More Info
Irregular heartbeats, also known as arrhythmias, can be concerning and uncomfortable, as you've described. The sensations of your heart racing or feeling like it's "skipping" beats can be alarming, especially when accompanied by symptoms like facial flushing, body heat, and subsequent chills. Understanding the underlying causes and potential solutions is crucial for managing these symptoms effectively.
Understanding Irregular Heartbeats
Arrhythmias can arise from various factors, including structural heart issues, electrolyte imbalances, hormonal changes, or even stress and anxiety. In your case, the fact that extensive testing, including 24-hour Holter monitoring, echocardiograms, and exercise stress tests, did not reveal any significant abnormalities is a positive sign. It suggests that your heart's structure and function are likely normal, which is reassuring.
The sensations you describe—such as the heart feeling like it's "knocking" or racing—could be attributed to a few types of arrhythmias. For instance, premature atrial contractions (PACs) or premature ventricular contractions (PVCs) are common benign arrhythmias that can cause such feelings. These are often harmless and can occur in healthy individuals, especially during periods of stress or fatigue.
Potential Causes
1. Stress and Anxiety: Emotional stress can lead to increased adrenaline levels, which may trigger palpitations. The feelings of heat and subsequent chills could be related to the body's stress response.
2. Electrolyte Imbalances: Low levels of potassium, magnesium, or calcium can affect heart rhythm. It's worth discussing with your healthcare provider whether a blood test to check these levels is appropriate.
3. Caffeine and Stimulants: High intake of caffeine or other stimulants can lead to irregular heartbeats. Monitoring your consumption of these substances may help.
4. Hormonal Changes: Fluctuations in hormones, particularly in women, can lead to palpitations. If you notice a pattern related to your menstrual cycle, this could be a factor.
5. Dehydration: Inadequate fluid intake can lead to electrolyte imbalances and increased heart rate. Ensuring proper hydration is essential.
Management Strategies
1. Lifestyle Modifications:
- Stress Management: Techniques such as yoga, meditation, or deep-breathing exercises can help reduce stress and anxiety, potentially alleviating symptoms.
- Dietary Adjustments: A balanced diet rich in fruits, vegetables, whole grains, and lean proteins can support overall heart health. Reducing caffeine and alcohol intake may also be beneficial.
- Regular Exercise: While it may seem counterintuitive, regular physical activity can strengthen the heart and improve overall cardiovascular health. Start with low-impact activities and gradually increase intensity as tolerated.
2. Medical Management:
- If your symptoms persist or worsen, discussing the possibility of medication with your healthcare provider may be warranted. Beta-blockers, for example, can help manage palpitations and reduce anxiety.
- Continuous monitoring may be necessary if your symptoms change or become more frequent.
3. Follow-Up: Regular follow-ups with a cardiologist can help track your symptoms and ensure that any new developments are addressed promptly. If symptoms persist despite lifestyle changes and reassurance, further investigation may be warranted.
Conclusion
While experiencing irregular heartbeats can be distressing, many cases are benign and manageable with lifestyle changes and stress management techniques. It's essential to maintain open communication with your healthcare provider to monitor your symptoms and adjust your management plan as needed. Remember that while your symptoms are real and can be uncomfortable, the absence of significant findings in your tests is a reassuring factor.
