After wisdom tooth extraction

Hello Doctor: I had my lower right wisdom tooth extracted last week, and I had my stitches removed on Saturday.
However, there is still a hole in my gums where food gets trapped.
Is this hole due to the dentist not stitching it properly, or will it heal on its own? Thank you.

Dr. Huang Zhicheng reply Dentistry

Hello, the healing of the extraction socket in the soft tissue typically takes about 1 to 2 weeks.
However, as you mentioned, the socket will only gradually heal completely once the underlying hard tissue, which is the bone, has also healed.
Wishing you peace and good health.
Sincerely, Dr.

More Info

After having a wisdom tooth extracted, it is common to have a healing period where the extraction site may appear as a "hole" or indentation in the gum. This is a normal part of the healing process and is not necessarily indicative of improper stitching by your dentist. The healing of the extraction site can take several weeks to months, depending on various factors such as the complexity of the extraction, your overall health, and how well you follow post-operative care instructions.

When a tooth is removed, the body begins the healing process immediately. Blood clots form in the socket, which is crucial for healing. Over time, this clot is replaced by granulation tissue, and eventually, new bone and gum tissue will fill in the socket. This process can take anywhere from a few weeks to several months, depending on the individual.

Here are some important points to consider regarding your healing process:
1. Normal Healing: It is normal to have a visible indentation or "hole" in the gum where the tooth was removed. This will gradually fill in as the tissue heals. The gum tissue will regenerate, and the bone will remodel itself over time.

2. Food Trapping: It is common for food particles to get trapped in the extraction site, especially in the early stages of healing. To manage this, you should avoid using straws, as the suction can dislodge the blood clot and delay healing. Instead, you can gently rinse your mouth with warm salt water after meals to help clean the area without causing irritation.

3. Pain and Discomfort: Some discomfort is expected after the extraction, and it may persist for a few days to a week. If you experience severe pain, swelling, or signs of infection (such as pus or fever), you should contact your dentist for further evaluation.

4. Follow-Up Care: It is essential to follow your dentist's post-operative care instructions. This may include taking prescribed medications, avoiding certain foods, and maintaining oral hygiene without disturbing the healing site. If you have concerns about the healing process, do not hesitate to reach out to your dentist for advice.

5. Timeframe for Healing: Generally, the soft tissue should begin to heal within a week or two, but complete healing of the bone and gum may take several months. Patience is key, as everyone's healing process is different.

6. Signs of Complications: While most healing processes go smoothly, be aware of signs that may indicate complications, such as persistent or worsening pain, excessive swelling, or a foul taste in your mouth. If you notice any of these symptoms, it is important to consult your dentist.

In summary, the "hole" you see is a natural part of the healing process after wisdom tooth extraction. It will gradually fill in as your body heals. Maintaining good oral hygiene and following your dentist's recommendations will help ensure a smooth recovery. If you have any concerns or if the healing process seems abnormal, do not hesitate to reach out to your dental professional for guidance.
