Menstrual issues vaginal bleeding
Hello, doctor.
I haven't had my period for two months (from the end of January to early March this year), and then in mid-March, I started experiencing continuous light vaginal bleeding.
I have not engaged in sexual intercourse and had normal menstrual cycles until last year.
This year is when I started noticing abnormalities.
I would like to ask the doctor what possible conditions could be causing this.
Thank you for your assistance.
Lena, 20~29 year old female. Ask Date: 2016/04/11

If the menstrual cycle becomes irregular in terms of duration or flow, it is often indicative of ovulatory disorders, and it is advisable to seek medical evaluation.

Abnormal vaginal bleeding can be a source of significant concern for many women, and understanding its potential causes is crucial for addressing any underlying health issues. Based on your description, it seems that you have experienced a notable change in your menstrual cycle, with a two-month absence of menstruation followed by persistent light bleeding. This situation can arise from various factors, and it is essential to consider both physiological and pathological causes.
Firstly, one of the most common reasons for abnormal vaginal bleeding, especially in women who have missed their periods, is hormonal imbalance. Hormonal fluctuations can occur due to several reasons, including stress, significant weight changes, excessive exercise, or underlying medical conditions such as polycystic ovary syndrome (PCOS). In your case, since you mentioned that you have not engaged in sexual activity, the likelihood of pregnancy-related issues is lower, but hormonal imbalances can still lead to irregular bleeding patterns.
Another potential cause of your symptoms could be related to the endometrium, the lining of the uterus. Conditions such as endometrial hyperplasia, which is an overgrowth of the endometrial lining often due to prolonged estrogen exposure without progesterone, can lead to abnormal bleeding. This condition is more common in women who are approaching menopause or those with irregular menstrual cycles.
Additionally, structural abnormalities in the reproductive system, such as uterine fibroids or polyps, can also lead to abnormal bleeding. These growths can cause changes in menstrual patterns, including prolonged or irregular bleeding. Although you mentioned that you have not had sexual intercourse, it is still important to rule out any structural issues through appropriate imaging studies, such as an ultrasound.
Infections can also be a contributing factor to abnormal vaginal bleeding. Conditions like cervicitis or pelvic inflammatory disease (PID) can lead to bleeding outside of the menstrual cycle. These infections can occur even in the absence of sexual activity, particularly if there are other risk factors present.
Given that you have experienced a significant change in your menstrual cycle and are now experiencing abnormal bleeding, it is crucial to seek medical evaluation. A healthcare provider may recommend a pelvic exam, blood tests to check hormone levels, and possibly imaging studies to assess the uterus and ovaries. A Pap smear may also be indicated to rule out any cervical abnormalities.
In summary, while there are numerous potential causes for your symptoms, including hormonal imbalances, structural abnormalities, and infections, a thorough evaluation by a healthcare professional is essential to determine the exact cause of your abnormal vaginal bleeding. It is important not to ignore these symptoms, as they can be indicative of underlying health issues that may require treatment. Please consider scheduling an appointment with a gynecologist to discuss your symptoms in detail and receive appropriate care.
