Painful Skin Bumps: Causes and When to Seek Help - Dermatology

There are cyst-like lumps on the skin that can rupture and cause pain?

Hello Dr.
Wu, in the past year or two, I have started to develop large, pimple-like lumps in the upper part of the gluteal fold, about the size of a thumb joint.
Initially, they felt hard to the touch, but gradually they became itchy and then painful, eventually breaking the skin.
I visited a clinic where the doctor said there was pus inside and helped drain it from the broken skin, prescribing an ointment for me to apply.
I thought that would resolve the issue, but the same area has recurred several times, and I have even had similar lumps near my shoulder blades.
Sometimes they resolve on their own, but other times they break open.
The pain during each recurrence is very bothersome, especially when sitting or lying down, as it increases the discomfort.
Could you please advise what this condition might be? Do I need to see a doctor? Thank you very much for taking the time to respond.

Without actually seeing the lesion, it cannot be confirmed.
If it is causing significant distress, it is advisable to seek medical attention as soon as possible.

More Info

Hello! Thank you for reaching out with your concerns regarding the painful skin bumps you've been experiencing. Based on your description, it sounds like you may be dealing with a condition known as hidradenitis suppurativa (HS), which is characterized by recurrent painful lumps or bumps in areas where skin rubs together, such as the groin, armpits, and under the breasts. These bumps can become inflamed, painful, and may even form abscesses that can drain pus, as you've described.

Possible Causes
1. Hidradenitis Suppurativa: This is a chronic skin condition that occurs when hair follicles become blocked, leading to inflammation and the formation of painful lumps. It is often associated with other conditions such as obesity, metabolic syndrome, and Crohn's disease.

2. Folliculitis: This is an inflammation of the hair follicles that can occur due to bacterial or fungal infections. It can cause red, itchy bumps that may become painful.

3. Boils (Furuncles): These are painful, pus-filled bumps that occur when hair follicles become infected. They can recur in the same area and may require drainage.

4. Other Skin Conditions: Conditions like abscesses, cysts, or even certain types of dermatitis can also present with similar symptoms.

When to Seek Help
Given that you have experienced recurrent episodes of these painful bumps, it is advisable to seek medical attention, especially since they have been persistent and painful. Here are some signs that indicate you should see a healthcare provider:
- Recurrent Infections: If the bumps keep coming back, it may indicate an underlying issue that needs to be addressed.

- Severe Pain: If the pain becomes unbearable or affects your daily activities, it's important to consult a doctor.

- Signs of Infection: If you notice increased redness, swelling, warmth, or fever, these could be signs of a more serious infection.

- Impact on Quality of Life: If the bumps are affecting your ability to sit, lie down, or perform daily activities comfortably, it’s time to seek help.

Management and Treatment
1. Topical Treatments: Your doctor may prescribe topical antibiotics or antiseptic creams to help manage the infection and inflammation.

2. Oral Antibiotics: If the bumps are recurrent and infected, oral antibiotics may be necessary to control the infection.

3. Lifestyle Modifications: Maintaining a healthy weight, wearing loose-fitting clothing, and practicing good hygiene can help reduce friction and irritation in affected areas.

4. Surgical Options: In some cases, if the bumps are severe or recurrent, surgical intervention may be necessary to drain abscesses or remove affected tissue.

5. Pain Management: Over-the-counter pain relievers can help manage discomfort associated with the bumps.

6. Follow-Up Care: Regular follow-up with a dermatologist or healthcare provider is crucial for managing chronic conditions like hidradenitis suppurativa.

Conclusion
In summary, the painful skin bumps you are experiencing could be indicative of hidradenitis suppurativa or another skin condition. Given the recurrent nature of your symptoms and the impact they have on your daily life, it is important to consult a healthcare provider for a thorough evaluation and appropriate management. Early intervention can help prevent complications and improve your quality of life. Please take care and seek medical attention as needed.
