Chronic Diarrhea: Possible Gastrointestinal Issues - Surgery

I am not sure if it is a gastrointestinal disease?

Hello, doctor.
I have some questions to ask you.
I have been experiencing chronic diarrhea for several years, with my stools often being loose or very thin.
Sometimes the color is quite dark.
After having a bowel movement, I notice a transparent, mucus-like substance on the toilet paper, and at times, there is a significant amount of blood that can flow for half an hour without stopping.
I would like to know what diseases these symptoms might indicate.
Thank you.

Wei, 30~39 year old female. Ask Date: 2016/04/13

Rectal bleeding is an abnormal symptom that can range from common conditions like hemorrhoids to more serious gastrointestinal diseases.
Based on your description, along with the presence of several other abnormal symptoms, it is strongly recommended that you visit a hospital's gastroenterology or surgical outpatient clinic for further evaluation, including a colonoscopy.

More Info

Chronic diarrhea can be a distressing condition, and your symptoms suggest that there may be underlying gastrointestinal issues that need to be addressed. Based on your description, several potential causes could be contributing to your symptoms, including inflammatory bowel disease (IBD), infections, food intolerances, or even structural abnormalities in the gastrointestinal tract.

Possible Causes of Chronic Diarrhea
1. Inflammatory Bowel Disease (IBD): This includes conditions such as Crohn's disease and ulcerative colitis. These diseases are characterized by chronic inflammation of the gastrointestinal tract, which can lead to symptoms such as persistent diarrhea, abdominal pain, and rectal bleeding. The presence of mucus or blood in your stool is particularly concerning and may indicate inflammation or ulceration in the intestines.

2. Infections: Chronic infections, whether bacterial, viral, or parasitic, can lead to ongoing diarrhea. These infections can sometimes be difficult to diagnose, especially if they are not typical or if they are caused by less common pathogens.

3. Food Intolerances or Allergies: Conditions such as lactose intolerance or celiac disease can cause chronic diarrhea. In these cases, the body reacts poorly to certain foods, leading to gastrointestinal symptoms. If you notice that your symptoms worsen after consuming specific foods, it may be worth investigating potential food intolerances.

4. Irritable Bowel Syndrome (IBS): This functional gastrointestinal disorder can cause chronic diarrhea, along with abdominal pain and changes in bowel habits. While IBS does not cause inflammation or damage to the intestines, it can significantly affect quality of life.

5. Structural Abnormalities: Conditions such as diverticulitis or colorectal polyps can also lead to chronic diarrhea and rectal bleeding. These conditions may require imaging studies or colonoscopy for proper diagnosis.

Recommended Next Steps
Given the severity and duration of your symptoms, it is crucial to seek medical evaluation. Here are some steps you should consider:
1. Consult a Gastroenterologist: A specialist can perform a thorough evaluation, including a detailed medical history, physical examination, and possibly laboratory tests or imaging studies.

2. Diagnostic Tests: You may need to undergo a colonoscopy to visualize the interior of your colon and rectum, which can help identify any inflammatory changes, polyps, or other abnormalities. Additionally, stool tests can help rule out infections or assess for inflammatory markers.

3. Dietary Assessment: Keeping a food diary to track your symptoms in relation to your diet can help identify any potential food triggers. An elimination diet may also be beneficial in identifying food intolerances.

4. Management of Symptoms: Depending on the diagnosis, treatment may include dietary modifications, medications to control inflammation or diarrhea, and possibly antibiotics if an infection is present.

Conclusion
Chronic diarrhea, especially when accompanied by blood or mucus, warrants prompt medical attention. It is essential to work closely with a healthcare provider to determine the underlying cause and develop an appropriate treatment plan. Early intervention can help prevent complications and improve your quality of life. Please do not hesitate to reach out to a healthcare professional for further evaluation and support.
