Abnormal bleeding

Hello, doctor.
I would like to ask about experiencing abnormal bleeding for the past two to three months after my menstrual period.
The first couple of times, it occurred four days after my period ended, and this month it happened about seven days after.
Previously, I was told that the abnormal bleeding was due to high stress levels, and I have been taking medication to manage it.
During that visit, the doctor performed an ultrasound.
I would like to know what other tests could be done besides the ultrasound in this situation.
Would it be better to undergo more detailed examinations? Thank you.

ting, 20~29 year old female. Ask Date: 2016/04/13

Abnormal post-menstrual bleeding can be concerning and may arise from various underlying causes. It is essential to understand the potential reasons for this bleeding and the appropriate diagnostic tests that can help identify the cause.

Firstly, abnormal bleeding after menstruation can be attributed to several factors, including hormonal imbalances, structural abnormalities in the reproductive system, or other medical conditions. Hormonal imbalances, often caused by stress, weight changes, or underlying health issues, can lead to irregular menstrual cycles and unexpected bleeding. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or even perimenopause can also contribute to these symptoms.

Structural abnormalities, such as fibroids, polyps, or endometrial hyperplasia, can cause abnormal bleeding. These conditions may lead to changes in the uterine lining, resulting in bleeding outside of the normal menstrual cycle. In some cases, infections or inflammatory conditions of the reproductive organs, such as pelvic inflammatory disease (PID), can also cause irregular bleeding.

Given your situation, where you have experienced abnormal bleeding after your menstrual period for the past few months, it is advisable to pursue further evaluation. While an ultrasound is a valuable tool for assessing the reproductive organs, there are additional tests that can provide more comprehensive insights into your condition.

1. Hormonal Testing: Blood tests can measure hormone levels, including estrogen, progesterone, thyroid hormones, and prolactin. These tests can help identify any hormonal imbalances that may be contributing to your abnormal bleeding.

2. Endometrial Biopsy: If there is a concern about the uterine lining, an endometrial biopsy may be performed. This procedure involves taking a small sample of the endometrium (the lining of the uterus) to check for abnormalities, such as hyperplasia or cancer.

3. Hysteroscopy: This procedure allows the doctor to view the inside of the uterus using a thin, lighted tube called a hysteroscope. It can help identify polyps, fibroids, or other abnormalities that may not be visible on an ultrasound.

4. Pap Smear: If you haven't had a recent Pap smear, it may be recommended to check for cervical abnormalities, including infections or precancerous changes.

5. Pelvic Exam: A thorough pelvic examination can help identify any physical abnormalities or signs of infection.

6. CT or MRI Scans: In certain cases, imaging studies like CT or MRI may be recommended to provide a more detailed view of the pelvic organs.

It's crucial to discuss your symptoms and concerns with your healthcare provider, who can guide you on the most appropriate tests based on your medical history and current symptoms. If stress has been identified as a contributing factor, addressing stress through lifestyle changes, therapy, or medication may also be beneficial.

In conclusion, while an ultrasound is a valuable diagnostic tool, additional tests such as hormonal assessments, endometrial biopsy, and hysteroscopy may provide further insights into the cause of your abnormal post-menstrual bleeding. Early diagnosis and intervention are key to managing any underlying conditions effectively. If you continue to experience abnormal bleeding or have any new symptoms, please seek medical attention promptly.
