Symptoms after surgery for diverticulitis?

Hello, my mother was hospitalized for diverticulitis in January of this year.
She was discharged later, but has since been experiencing abdominal pain and has had fecal matter leaking from her vagina.
She has been readmitted to the hospital multiple times, and the doctors have not indicated any major issues, continuing to administer antibiotics and provide inpatient treatment.
She has now been discharged again, but is currently experiencing vaginal bleeding.
Is this a normal occurrence? Besides another surgery, are there other solutions to address these primary symptoms?

Dr. Chen Qifeng reply Gastroenterology and Hepatology

It needs to be further clarified whether the substance being expelled from the vagina is feces.
If so, there may be the formation of a fistula (but more complete information is required).

Post-surgery symptoms following a diverticulitis operation can be complex and concerning, especially when they involve abdominal pain and unusual bleeding. In your mother's case, the persistent abdominal pain and the passage of stool from the vagina are alarming symptoms that warrant immediate attention.
Diverticulitis occurs when small pouches (diverticula) in the colon become inflamed or infected. Surgical intervention, such as a resection of the affected part of the colon, is sometimes necessary when complications arise. Post-operative symptoms can vary widely, but they should not include significant bleeding from the vagina or the passage of stool through the vaginal canal.

The symptoms your mother is experiencing—abdominal pain and vaginal bleeding—could indicate several potential complications. One possibility is the formation of a fistula, which is an abnormal connection between the colon and the vagina. This can occur after surgery, especially if there was significant inflammation or infection during the diverticulitis episode. A fistula can lead to the passage of stool through the vagina, which is not only uncomfortable but can also lead to further complications, including infections.

The presence of blood in the vaginal discharge is particularly concerning. While some bleeding can occur due to various benign reasons, such as hormonal changes or minor trauma, the combination of vaginal bleeding and abdominal pain following a diverticulitis surgery raises the suspicion of a more serious issue, such as a fistula or other surgical complications.
It is crucial for your mother to seek immediate medical attention. A thorough evaluation by a healthcare professional, including imaging studies like a CT scan or MRI, may be necessary to assess for the presence of a fistula or other complications. If a fistula is confirmed, surgical intervention may be required to repair it.
In terms of management, antibiotics may help if there is an infection, but they will not resolve a fistula. If surgery is necessary, the approach will depend on the specific circumstances, including the location and size of the fistula, as well as your mother’s overall health.

In summary, the symptoms your mother is experiencing are not normal and should not be ignored. It is essential to consult with her healthcare provider as soon as possible to determine the underlying cause of her symptoms and to discuss the appropriate treatment options. Early intervention can prevent further complications and improve her quality of life.
