Abdominal pain with diarrhea and bleeding?

Hello Doctor: I have been experiencing lower abdominal pain for several days (just below the navel).
My bowel movements started off loose and unformed (the color was dark brown).
Usually, after I finish, my stomach still hurts, and when I go again, the stool is mostly a bright yellow watery consistency.
Two days ago, I noticed bright red blood in my stool.
I saw a doctor who prescribed me anti-diarrheal medication and probiotics.
After resting for a day, I did not have a bowel movement yesterday, but today my stool was formed and clay-colored, although there is still a small amount of dark red blood mixed in.
Doctor, could this bleeding be caused by excessive diarrhea from enteritis, or do I have hemorrhoids?

Hello: The likelihood of bleeding caused by enteritis is relatively high.
The bleeding from a hemangioma is often bright red.

Understanding abdominal pain and blood in stool can be concerning, and it's essential to consider various potential causes. Based on your description, you have been experiencing lower abdominal pain, changes in stool consistency and color, and the presence of blood in your stool. These symptoms can arise from several conditions, and it's crucial to evaluate them carefully.

Firstly, the presence of abdominal pain, especially localized below the umbilicus (belly button), can indicate several gastrointestinal issues. Common causes include infections, inflammatory bowel diseases, and functional gastrointestinal disorders. Given that your stool has changed from loose and discolored to more formed but still contains blood, this suggests a possible inflammatory process in the intestines.

1. Gastroenteritis: This is an inflammation of the stomach and intestines, often caused by viral or bacterial infections. Symptoms typically include diarrhea, abdominal pain, and sometimes fever. If you had a recent episode of diarrhea, it could lead to irritation of the intestinal lining, which might cause blood in the stool.

2. Inflammatory Bowel Disease (IBD): Conditions like Crohn's disease or ulcerative colitis can cause abdominal pain, diarrhea, and blood in the stool. These diseases are characterized by chronic inflammation of the gastrointestinal tract and may require further diagnostic testing, such as colonoscopy, to confirm.

3. Hemorrhoids: If the blood is bright red and appears on the surface of the stool or in the toilet bowl, it could be due to hemorrhoids, which are swollen blood vessels in the rectal area. They can cause pain and bleeding, especially after bowel movements.

4. Anal Fissures: These are small tears in the lining of the anus, often caused by passing hard stools. They can lead to sharp pain during bowel movements and may also cause bleeding.

5. Diverticulitis: This condition occurs when small pouches in the colon become inflamed or infected. Symptoms can include lower abdominal pain, changes in bowel habits, and sometimes blood in the stool.

6. Colorectal Cancer: While less common, especially in younger individuals, it is essential to consider this possibility, particularly if there are other risk factors or a family history of colorectal cancer.

Given that you have already consulted a doctor and received treatment with antidiarrheal medication and probiotics, it is a positive step. However, the persistence of symptoms, especially the presence of blood, warrants further investigation. It is advisable to follow up with your healthcare provider for additional tests, such as stool studies, imaging, or possibly a colonoscopy, to rule out more serious conditions.

In summary, while your symptoms could be related to a transient gastrointestinal infection or irritation, the presence of blood in your stool is a sign that should not be ignored. It is crucial to monitor your symptoms closely and seek further medical evaluation if they persist or worsen. Early diagnosis and treatment can significantly improve outcomes and help address any underlying issues effectively.
