Menstrual discomfort

*I missed my period in March.
On March 25th, I experienced vaginal itching and frequent urination for over a week.
I’m not sure what’s going on! I was wondering if I might be pregnant.
My period came from April 3rd to April 9th and ended on the 9th.
After that, I thought my cycle had finally ended, but unexpectedly, it returned just three days later.
This time, the flow is very light.
What could this symptom indicate? I often have my second period during my fertile window.*

QQ, 20~29 year old female. Ask Date: 2016/04/20

Dr. Lü Lizheng reply Obstetrics and Gynecology

The response may indicate irregular menstruation; other possibilities include tumors, inflammation, pregnancy, and polyps.

Irregular periods can be a source of concern for many women, and understanding the symptoms and potential underlying issues is crucial for maintaining reproductive health. Based on the symptoms you've described, including vaginal itching, frequent urination, and the irregularity of your menstrual cycle, there are several factors to consider.

Firstly, it's important to note that menstrual cycles can vary significantly from woman to woman and can be influenced by various factors, including hormonal changes, stress, lifestyle, and underlying health conditions. A typical menstrual cycle ranges from 21 to 35 days, with bleeding lasting between 2 to 7 days. However, cycles can become irregular due to several reasons, including hormonal imbalances, stress, significant weight changes, or medical conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders.

In your case, the vaginal itching and frequent urination you experienced in March could suggest a possible infection, such as a yeast infection or a urinary tract infection (UTI). These conditions can sometimes cause discomfort and may also affect your menstrual cycle. If you suspect an infection, it is advisable to consult a healthcare provider for proper diagnosis and treatment.

The fact that you experienced a menstrual period from April 3 to April 9, followed by another light period shortly after, could indicate a few possibilities. One common explanation for light bleeding after a period is known as "breakthrough bleeding," which can occur due to hormonal fluctuations, especially if you are not on hormonal birth control. Additionally, if you are experiencing frequent periods, it could be a sign of anovulation, where the ovaries do not release an egg, leading to irregular bleeding patterns.

If you are concerned about the possibility of pregnancy, it is essential to take a home pregnancy test, especially if you have had unprotected intercourse. Pregnancy can sometimes present with symptoms similar to those of premenstrual syndrome (PMS), including breast tenderness, mood swings, and changes in appetite. However, if the test is negative and your periods remain irregular, further evaluation may be necessary.

Given your symptoms and the changes in your menstrual cycle, it would be prudent to schedule an appointment with a healthcare provider. They can perform a thorough evaluation, which may include a physical examination, blood tests to check hormone levels, and possibly imaging studies to assess your reproductive organs. This comprehensive approach will help identify any underlying issues that may be contributing to your irregular periods and other symptoms.

In summary, irregular periods can be caused by a variety of factors, including hormonal imbalances, infections, and lifestyle changes. The symptoms you are experiencing warrant further investigation to rule out any serious conditions and to provide appropriate treatment. Consulting with a healthcare provider will ensure that you receive the necessary care and guidance to manage your reproductive health effectively.
