Irregular abnormalities of the penile skin?
During middle school, there was an incident where I experienced an erection during recess.
I leaned against a concrete wall to enjoy the scenery, but a classmate jokingly bumped into me, causing my penis to hit the wall and sustain an injury.
I felt a sharp pain and noticed a small wound that bled slightly, similar to a minor scrape, which I didn't think much of at the time.
However, over the past ten years, possibly due to weather or other factors, that wound occasionally reopens, creating a small fissure with some blood and mild itching or a slight stinging sensation.
Sometimes, the area has what appears to be white flakes, resembling mold on cake, but not as severe.
At other times, it appears completely normal with no issues.
I've considered the possibility of a fungal infection and have applied various ointments, but none have been effective.
What could be the problem, and how can it be effectively treated?

Without actually seeing the lesion, it cannot be confirmed.
If it is causing significant distress, it is advisable to seek medical attention as soon as possible.

Understanding irregular skin issues on the penis can be complex, especially when there is a history of trauma, as you described. Based on your account, it seems that you may be experiencing a combination of skin irritation, possible fungal infection, and perhaps a form of dermatitis or scarring from the initial injury.
Firstly, the initial trauma you experienced over a decade ago could have led to some form of scarring or chronic irritation in that area. Scarring can sometimes cause the skin to be more sensitive or prone to cracking, especially if the skin is dry or if there are environmental factors at play, such as temperature changes or humidity levels. The fact that you occasionally notice a small crack that bleeds slightly and itches suggests that the skin in that area may not be healing properly or is prone to irritation.
The presence of white flakes or "dandruff-like" skin could indicate a few possibilities. One common issue is seborrheic dermatitis, which can occur in various areas of the body, including the genital region. This condition is characterized by red, flaky patches and can be exacerbated by stress, hormonal changes, or environmental factors. Another possibility is a fungal infection, such as candidiasis, which can thrive in warm, moist areas and may present with itching, irritation, and flaking skin.
Given that you've tried topical treatments without success, it may be time to consider a few different approaches:
1. Consult a Dermatologist: It's crucial to see a healthcare professional who specializes in skin conditions. They can provide a proper diagnosis and may perform tests to rule out infections or other skin conditions. A dermatologist can also prescribe stronger topical treatments or oral medications if necessary.
2. Maintain Hygiene: Ensure that the area is kept clean and dry. Use mild, fragrance-free soaps and avoid harsh chemicals that could irritate the skin further. After bathing, gently pat the area dry rather than rubbing it.
3. Moisturize: If the skin is dry, applying a gentle, hypoallergenic moisturizer can help maintain skin hydration. Look for products that are specifically designed for sensitive skin or for use in the genital area.
4. Avoid Irritants: Be mindful of any products you apply to the area, including soaps, lotions, or even laundry detergents that may cause irritation. Opt for products that are free from fragrances and dyes.
5. Consider Antifungal Treatments: If a fungal infection is suspected, over-the-counter antifungal creams may be beneficial. However, it’s best to consult with a doctor before starting any treatment to ensure it’s appropriate for your condition.
6. Monitor Symptoms: Keep track of any changes in your symptoms, including triggers that may exacerbate the condition. This information can be valuable for your healthcare provider in determining the best course of action.
7. Lifestyle Factors: Consider factors such as diet, stress levels, and overall health, as these can impact skin conditions. A balanced diet rich in vitamins and minerals can support skin health.
In summary, while the initial injury may have set the stage for ongoing skin issues, a combination of proper medical evaluation, good hygiene practices, and appropriate topical treatments can help manage and potentially resolve the symptoms you are experiencing. Always prioritize consulting with a healthcare professional for personalized advice and treatment options tailored to your specific situation.
