Eye pain and headache (forehead + temples)

Hello doctor, on April 4th, due to working the night shift, I hardly slept all night.
That morning, I started experiencing pain in my eyeball, which was intense enough to cause pain in my forehead and temples as well.
Initially, the pain was in my left eye, left forehead, and left temple, but later it shifted to my right eye along with forehead and temple pain.
(I had a cold before April 4th.) I previously consulted a doctor who diagnosed it as a complication from the cold.
However, after another week, my eye and head pain have not improved.
I would like to ask what other symptoms this might indicate?

QQ, 20~29 year old female. Ask Date: 2016/04/21

Dr. Wang Zhangji reply Ophthalmology

It may be due to muscle fatigue or eye pressure issues caused by frequent late nights.

Hello! It sounds like you are experiencing a complex interplay of symptoms that may be related to several underlying issues, particularly following a period of sleep deprivation. Your symptoms of eye pain, headaches, and the shifting nature of the pain from one side of your head to the other can be indicative of a few different conditions.

Firstly, sleep deprivation itself can lead to a variety of physical symptoms, including headaches and eye discomfort. When you do not get enough sleep, your body can become stressed, leading to muscle tension, particularly in the neck and head, which can contribute to tension-type headaches. These headaches often present as a dull, aching sensation and can be accompanied by tightness or pressure around the forehead, temples, or back of the head.

The fact that your eye pain is associated with headaches could suggest a few possibilities. One common condition is a tension headache, which can cause pain around the eyes and forehead. Additionally, if the pain is more severe and accompanied by visual disturbances, it could indicate a migraine. Migraines can sometimes be triggered by stress, lack of sleep, or even changes in weather, and they can cause significant pain that may be unilateral (affecting one side of the head) and pulsating in nature.

Another consideration is that your symptoms may be related to sinus issues, especially since you mentioned having a cold prior to the onset of your symptoms. Sinusitis can cause facial pain, pressure around the eyes, and headaches, particularly if there is inflammation or infection in the sinus cavities. This could explain the eye pain and the headache that seems to radiate from the forehead and temples.

Given that your symptoms have persisted for over a week without improvement, it is advisable to seek further evaluation from a healthcare professional. A visit to a neurologist or an ophthalmologist may be warranted to rule out any serious underlying conditions. They may perform a thorough examination, including checking your eye health, assessing for any signs of sinus infection, and evaluating your neurological status.

In the meantime, there are some self-care strategies you can employ to help alleviate your symptoms. Ensuring you get adequate rest and managing stress through relaxation techniques such as deep breathing, meditation, or gentle yoga can be beneficial. Staying hydrated and using over-the-counter pain relief medications, such as ibuprofen or acetaminophen, may also help reduce headache pain. If you suspect that your eye pain is related to dryness or strain, using artificial tears or taking regular breaks from screens can provide relief.

Lastly, if you have a history of anxiety, it may also play a role in your symptoms. Anxiety can exacerbate physical symptoms, including headaches and eye discomfort. Addressing anxiety through therapy, mindfulness practices, or medication (if prescribed) can also be an important part of managing your overall health.

In summary, your symptoms could be due to a combination of tension headaches, sinus issues, or even migraines, particularly following sleep deprivation. It is essential to consult with a healthcare professional for a comprehensive evaluation to determine the exact cause and appropriate treatment for your condition. Take care, and I hope you find relief soon!
