Lower abdominal bleeding?
Hello Doctor, I have been experiencing bleeding this year, sometimes for over ten days.
Previously, I bled from March 28 to April 5, and then again from April 19 to today, the 22nd.
I have some light bleeding because I am taking Gestrinone 2.5 mg.
Even while on this medication, I still experience menstrual bleeding.
The doctor has advised me to continue taking this medication and asked if I want to keep taking it.
I find my menstrual periods very uncomfortable, and I need to apply heat for a long time.
The amount of bleeding is similar to urinary incontinence, which is quite alarming.
My mother wants me to tell the doctor to stop the medication, but I do not want to stop it.
Additionally, I have a uterine fibroid, although I have had surgery for it.
A few days ago, I had a follow-up appointment where they tested my urine and diagnosed me with mild urethritis.
I did not take antibiotics because they have not been effective for me in the past, and I have taken quite a few antibiotics before.
Therefore, I have been drinking plenty of water due to my ongoing bleeding symptoms, even though it is minimal.
I would like to ask for your opinion, Doctor.
Previously, I took Gestrinone 2.5 mg twice a week, but starting this month, the dosage has increased to three times a week.
Given my situation, what tests would be advisable? I am quite overweight and am concerned that some tests may not be suitable for me.
However, I would appreciate your recommendations.
The gynecologist only conducted a urine test because I am still unclear about the cause of the bleeding—whether it is menstrual, due to inflammation, or caused by other issues.

More Info
Abnormal bleeding can be a significant concern for many women, and understanding its causes and treatment options is crucial for managing symptoms and maintaining overall health. Based on your description, it appears that you are experiencing prolonged and irregular bleeding, which can be distressing and may require a comprehensive evaluation.
Causes of Abnormal Bleeding
1. Hormonal Imbalances: One of the most common causes of abnormal bleeding is hormonal imbalance. This can occur due to various factors, including stress, changes in weight, or underlying medical conditions. In your case, the use of Gestrinone, a medication that affects hormonal levels, may contribute to your bleeding patterns. While it is often prescribed for conditions like endometriosis or fibroids, it can also lead to irregular bleeding as a side effect.
2. Uterine Fibroids: You mentioned having uterine fibroids, which are non-cancerous growths in the uterus that can cause heavy menstrual bleeding, prolonged periods, and spotting between periods. Even after surgical intervention, fibroids can recur or new ones can develop, leading to ongoing symptoms.
3. Infections: Urinary tract infections (UTIs) or pelvic inflammatory disease (PID) can also cause abnormal bleeding. You noted experiencing mild urinary symptoms, which could indicate an underlying infection that might be contributing to your bleeding.
4. Structural Abnormalities: Conditions such as polyps or adenomyosis can also lead to abnormal bleeding. These conditions may require imaging studies like ultrasound or hysteroscopy for proper diagnosis.
5. Other Medical Conditions: Conditions such as thyroid disorders, clotting disorders, or even certain cancers can lead to abnormal bleeding. It is essential to rule these out, especially if the bleeding is persistent and unexplained.
Treatment Options
1. Medication Adjustment: Since you are currently taking Gestrinone, it is essential to discuss with your healthcare provider whether the dosage needs adjustment or if an alternative medication might be more suitable. Sometimes, a different hormonal treatment or the addition of a progestin can help regulate bleeding.
2. Monitoring and Follow-Up: Regular follow-up appointments with your gynecologist are crucial. They may recommend periodic ultrasounds to monitor the size and number of fibroids, as well as blood tests to check for anemia or hormonal levels.
3. Lifestyle Modifications: Maintaining a healthy weight and managing stress can positively impact hormonal balance. Regular exercise and a balanced diet may help alleviate some symptoms.
4. Surgical Options: If fibroids are determined to be the primary cause of your bleeding and conservative treatments are ineffective, surgical options such as myomectomy (removal of fibroids) or hysterectomy (removal of the uterus) may be considered.
5. Addressing Infections: If a urinary tract infection or other infection is diagnosed, appropriate antibiotics should be prescribed. It’s important to complete the course of antibiotics as directed, even if symptoms improve.
6. Further Testing: Given your concerns about the cause of your bleeding, your doctor may recommend additional tests such as a pelvic ultrasound, hysteroscopy, or endometrial biopsy to evaluate the uterine lining and rule out any serious conditions.
Conclusion
It is understandable to feel anxious about ongoing abnormal bleeding, especially with a history of fibroids and hormonal treatment. Open communication with your healthcare provider is essential to tailor a treatment plan that addresses your symptoms while considering your overall health and preferences. If you feel that your current treatment is not effective, do not hesitate to seek a second opinion or discuss alternative options with your doctor. Remember, your health and comfort are paramount, and there are various avenues to explore for relief and management of your symptoms.
