Widespread Skin Redness and Itching: Possible Internal Causes - Dermatology

Widespread skin redness, swelling, and itching?

Hello, I would like to ask about my uncle who has red, swollen wounds all over his body that are very itchy, as if something is burrowing into his skin.
He also has white, pus-like secretions and even reports that they can burst while he is sleeping, which he can feel.
I realize my description may be somewhat abstract, but he has visited many dermatology clinics, and they all say there is not a significant issue, only prescribing topical medications that have not improved his condition.
I would like to understand if this situation could potentially be a skin issue triggered by underlying health problems.
Thank you.

Ms. Chen, 30~39 year old female. Ask Date: 2016/04/25

Without actually seeing the lesion, it is difficult to make a definitive diagnosis.
If you are very concerned, please seek medical attention as soon as possible.

The symptoms your uncle is experiencing, including widespread skin redness, itching, and the presence of pustules, can indeed be indicative of underlying internal issues. While it is essential to have a thorough examination by a healthcare professional to determine the exact cause, I can provide some insights into potential internal causes that may lead to such skin manifestations.

1. Allergic Reactions: One of the most common internal causes of widespread skin issues is an allergic reaction. This could be due to food, medications, or environmental allergens. When the body encounters an allergen, it can trigger a histamine response, leading to redness, itching, and the formation of hives or pustules. If your uncle has recently changed his diet or started new medications, these could be potential culprits.

2. Autoimmune Disorders: Conditions such as lupus or psoriasis can cause widespread skin symptoms. Autoimmune diseases occur when the body's immune system mistakenly attacks its own tissues. In the case of psoriasis, for example, it can lead to red, scaly patches on the skin, which can be itchy and uncomfortable. If there are other systemic symptoms, such as joint pain or fatigue, it would be prudent to investigate autoimmune conditions further.

3. Infections: Both bacterial and viral infections can lead to skin issues. For instance, folliculitis (infection of hair follicles) can cause red, itchy bumps that may resemble pustules. Additionally, systemic infections can manifest as skin symptoms, especially if the immune system is compromised.

4. Internal Organ Issues: Conditions affecting the liver, kidneys, or gastrointestinal tract can also lead to skin changes. For example, liver dysfunction can cause jaundice and itching due to the accumulation of bile salts in the bloodstream. Similarly, gastrointestinal issues, such as celiac disease or inflammatory bowel disease, can lead to skin manifestations like dermatitis herpetiformis.

5. Hormonal Imbalances: Hormonal changes can also affect the skin. Conditions such as hyperthyroidism or hormonal fluctuations during puberty or menopause can lead to increased oil production, resulting in acne-like pustules and skin irritation.

6. Stress and Psychological Factors: Stress can exacerbate skin conditions, leading to flare-ups of existing issues or the development of new ones. Conditions like eczema or psoriasis can worsen with stress, leading to increased itching and redness.

Given that your uncle has seen multiple dermatologists without significant improvement, it may be beneficial for him to consult with a specialist who can evaluate his overall health, including a thorough review of his medical history, lifestyle, and any other symptoms he may be experiencing. A multidisciplinary approach may be necessary, involving dermatologists, allergists, and possibly rheumatologists or internists, to get to the root of the problem.

In the meantime, managing symptoms with topical treatments, antihistamines, and maintaining a healthy skin care routine can help alleviate some discomfort. It is also advisable to keep a diary of any potential triggers, including foods, activities, or environmental changes, which may help identify patterns that could lead to a diagnosis.

In conclusion, while the skin is often the most visible indicator of health, it can reflect deeper internal issues. A comprehensive evaluation is crucial for proper diagnosis and treatment. Encourage your uncle to persist in seeking answers until he finds a healthcare provider who can address his concerns thoroughly.
