It has been 6 weeks since the hemorrhoid surgery?
Dear Dr.
Ke: I have mixed hemorrhoids, and having 11 of them is quite severe, as there is a ring of them around the anal area.
It has been 6 weeks since my surgery, but I still notice blood dripping after each bowel movement.
Previously, the blood was dark red, but now it appears red (not bright red, more like a peachy red).
My stools are not hard, but I do experience anal pain during bowel movements.
Today, I had a bit of an upset stomach and diarrhea, and I also noticed a few drops of blood then.
Why does diarrhea also cause bleeding? I have been using suppositories, but there is still bleeding.
There is no bleeding when I am not having a bowel movement.
Is this normal? Thank you for your response, Dr.
Ke.

Dr. Ke Fangxu reply Surgery
Generally, it takes about a month after surgery to return to normal.
However, intermittent mild bleeding is not a concern.
It is recommended to improve your bowel habits and avoid diarrhea.
If bleeding persists, it is best to consult the original surgeon for further evaluation.

After undergoing surgery for mixed hemorrhoids, it's understandable to have concerns regarding post-operative symptoms, especially when experiencing blood after bowel movements and associated pain. Let's break down your situation and address your concerns.
Firstly, it's important to note that some bleeding after hemorrhoid surgery can be expected, especially in the initial weeks following the procedure. However, the fact that you are still experiencing bleeding six weeks post-surgery warrants further investigation. The color of the blood you described—transitioning from dark red to a brighter, peachy red—can indicate that the source of the bleeding may be closer to the anal opening, which is often the case with hemorrhoids. Bright red blood typically suggests that the bleeding is from a source that is not too far from the rectum, such as a hemorrhoid or a fissure.
The pain you are experiencing during bowel movements could be attributed to several factors. Post-surgical pain is common, and it may be exacerbated by the presence of fissures, which are small tears in the lining of the anus that can occur due to straining during bowel movements. Even if your stools are not particularly hard, the act of passing them can still cause discomfort, especially if the anal area is inflamed or sensitive from the surgery.
Regarding your question about experiencing bleeding during diarrhea, this can happen as well. Diarrhea can irritate the anal region, leading to increased friction and pressure, which may cause existing hemorrhoids or fissures to bleed. It's crucial to maintain a soft stool consistency to minimize straining, which can aggravate the area.
Here are some recommendations to help manage your symptoms:
1. Dietary Adjustments: Ensure you are consuming a high-fiber diet to promote softer stools. Foods rich in fiber include fruits, vegetables, whole grains, and legumes. Staying well-hydrated is also essential.
2. Sitz Baths: Taking warm sitz baths can help soothe the anal area and promote healing. This can be done several times a day, especially after bowel movements.
3. Topical Treatments: Continue using any prescribed topical medications or suppositories as directed by your physician. These can help reduce inflammation and promote healing.
4. Avoid Straining: When you feel the urge to have a bowel movement, try to go as soon as possible. Avoid straining, as this can worsen your symptoms.
5. Follow-Up with Your Surgeon: Since you are experiencing ongoing symptoms, it is crucial to follow up with your surgeon or a gastroenterologist. They may want to perform an examination to rule out any complications, such as residual hemorrhoids, anal fissures, or other issues that may require further treatment.
6. Pain Management: If you are experiencing significant pain, discuss pain management options with your healthcare provider. They may recommend over-the-counter pain relievers or prescribe medication if necessary.
In summary, while some bleeding and discomfort can be normal after hemorrhoid surgery, persistent symptoms should not be ignored. It is essential to communicate openly with your healthcare provider about your symptoms and concerns to ensure appropriate management and care. Your health and comfort are paramount, and addressing these issues promptly can lead to better outcomes and improved quality of life.
