Persistent chest tightness?
Hello, doctor.
I have been experiencing chest tightness and a desire to take deep breaths, which usually occurs during seasonal changes.
I typically don't have any issues, but this has been persistent for nearly two weeks now, almost continuously throughout the day, with occasional hours of inexplicable improvement.
About 3 to 4 years ago, I underwent an echocardiogram, a 24-hour Holter monitor, and a stress test, all of which were normal.
I also had a stress test about 2 years ago, which was normal as well.
I do not experience chest pain while jogging; in fact, I find my breathing becomes easier and more comfortable while running.
Here is my medical history: I have a history of spontaneous pneumothorax on both sides, with surgery performed on the right side, and I have not had any recurrences for at least 6 years.
Additionally, I have mild mitral valve prolapse.
Could you please advise me on what might be the issue? Do I need to undergo further testing? Thank you for taking the time to respond.

More Info
Persistent chest tightness can be a concerning symptom, especially when it lasts for an extended period, as you described. Given your medical history, including spontaneous pneumothorax and mild mitral valve prolapse, it is essential to approach this situation with caution and thoroughness.
Possible Causes of Chest Tightness
1. Musculoskeletal Issues: Often, chest tightness can stem from musculoskeletal problems, such as muscle strain or costochondritis (inflammation of the cartilage connecting the ribs to the sternum). This is particularly common during seasonal changes when people may engage in different physical activities or experience changes in posture.
2. Anxiety and Stress: Psychological factors can also contribute significantly to sensations of chest tightness. Anxiety and stress can lead to hyperventilation, which may cause a feeling of tightness in the chest. Given that you mentioned a history of anxiety, this could be a contributing factor.
3. Respiratory Conditions: Conditions such as asthma or chronic obstructive pulmonary disease (COPD) can cause chest tightness, especially if there is a change in weather or allergens in the environment. Since you noted that your symptoms seem to correlate with seasonal changes, this could be worth considering.
4. Cardiac Issues: Although you have had normal cardiac evaluations in the past, it is crucial to remain vigilant. Conditions such as angina or other cardiac issues can sometimes present as chest tightness. Given your history of mitral valve prolapse, it is essential to monitor any changes in your symptoms closely.
5. Gastroesophageal Reflux Disease (GERD): Acid reflux can cause a sensation of tightness in the chest, often mistaken for cardiac-related pain. If you have any accompanying symptoms like heartburn or regurgitation, this could be a potential cause.
When to Seek Help
Given that your symptoms have persisted for nearly two weeks and are almost continuous, it is advisable to seek medical attention. Here are some specific indicators that warrant immediate evaluation:
- Worsening Symptoms: If the tightness increases in severity or frequency.
- Accompanying Symptoms: If you experience additional symptoms such as shortness of breath, dizziness, palpitations, or radiating pain to the arms, neck, or jaw.
- Changes in Activity: If you notice that the tightness occurs during physical activity or at rest, particularly if it is new or different from your previous experiences.
- History of Cardiac Issues: Given your history of spontaneous pneumothorax and mitral valve prolapse, it is prudent to have a healthcare provider evaluate your symptoms.
Recommended Actions
1. Consult a Healthcare Provider: Given your history and the persistence of your symptoms, it would be wise to consult a cardiologist or a pulmonologist. They may recommend further tests, such as a repeat echocardiogram, stress testing, or pulmonary function tests, to rule out any underlying issues.
2. Monitor Symptoms: Keep a diary of your symptoms, noting when they occur, their duration, and any associated activities or stressors. This information can be invaluable for your healthcare provider.
3. Manage Stress and Anxiety: If anxiety is a contributing factor, consider engaging in relaxation techniques such as deep breathing exercises, yoga, or mindfulness meditation. These practices can help alleviate some of the physical symptoms associated with anxiety.
4. Lifestyle Modifications: Ensure you are maintaining a healthy lifestyle, including regular exercise, a balanced diet, and adequate hydration. Avoiding known triggers, such as caffeine or smoking, can also help manage symptoms.
In conclusion, while there can be various causes for your persistent chest tightness, it is essential to approach this symptom with a comprehensive evaluation. Given your medical history and the duration of your symptoms, seeking professional medical advice is the best course of action to ensure your health and peace of mind.
