Right testicular pain with a hard lump!

Hello, I would like to ask about my husband who had a vasectomy a year ago.
Since then, he has been experiencing pain in his right testicle, which has recently become more severe.
There is also a hard lump on the testicle that is painful to touch.
Could this be related to the surgery? Thank you for your response!

Dr. Li Xinyi reply Urology

It is recommended to see a urologist for an examination, including a urinalysis and a scrotal ultrasound.

After a vasectomy, it is not uncommon for some men to experience discomfort or pain in the testicles, as well as the development of lumps. In your husband's case, the right testicle pain and the hard lump that has developed could be attributed to several potential causes, which I will outline below.

1. Post-Vasectomy Pain Syndrome (PVPS): This condition can occur in some men after a vasectomy. It is characterized by chronic pain in the testicles or scrotum that persists long after the procedure. The pain can be due to nerve damage, inflammation, or changes in the way sperm is stored and transported after the vasectomy.

2. Sperm Granuloma: After a vasectomy, sperm can leak from the cut ends of the vas deferens, leading to a localized inflammatory response. This can form a lump known as a sperm granuloma, which can be painful. These lumps are typically benign but can cause discomfort.

3. Epididymitis: This is an inflammation of the epididymis, the tube that carries sperm from the testicle. It can occur due to infection or trauma and may present with pain and swelling in the affected testicle. If the pain is accompanied by redness, swelling, or fever, it may indicate an infection that requires medical attention.

4. Varicocele: This condition involves enlarged veins within the scrotum and can lead to a feeling of heaviness or pain in the testicle. It is more common on the left side but can occur on the right as well.

5. Testicular Torsion: Although this is a medical emergency and typically presents with sudden severe pain, it is important to consider if the pain is acute and severe. Testicular torsion occurs when the spermatic cord becomes twisted, cutting off blood supply to the testicle.

6. Hernia: Inguinal hernias can sometimes present with pain in the testicular area, especially if there is a bulge or lump in the groin. This occurs when tissue protrudes through a weak spot in the abdominal muscles.

Given that your husband has been experiencing increasing pain and has developed a hard lump, it is crucial that he seeks medical evaluation as soon as possible. A healthcare provider will likely perform a physical examination and may order imaging studies, such as an ultrasound, to assess the lump and determine the underlying cause of the pain.

In summary, while some discomfort after a vasectomy can be normal, the increasing severity of pain and the presence of a hard lump warrant further investigation. It is essential to rule out any serious conditions and to receive appropriate treatment to alleviate the symptoms. Please encourage your husband to consult with a urologist or his primary care physician for a thorough evaluation and management plan.
