Doctor, I would like to ask about CO2 fractional laser treatment for moles.
Is it possible for raised moles to become completely flat after the laser treatment? Can CO2 fractional laser be used to flatten hypertrophic scars? What about mature hypertrophic scars? Is there a chance that some moles will not completely disappear after CO2 fractional laser treatment? I have had three treatments for a mole; during the first two sessions, it seemed to gradually shrink, but after the third session, it appeared to be the same size as before.
Why is that? Could it be because the mole is deeper? Is it possible that after several treatments, the mole still won't disappear, or will it eventually fade with continued treatment? This mole is approximately 3-4 mm in size and 1-2 mm raised, and it has some hair growing from it.

Dr. Wu Yuxin reply Dermatology
There is a possibility of residual pigmentation after laser mole removal.
If there is no improvement after 3-4 sessions of laser treatment, surgical intervention should be considered.

CO2 laser treatments are widely used in dermatology for various skin conditions, including the flattening of raised moles (nevi) and hypertrophic scars. The effectiveness of CO2 laser therapy in achieving a flatter appearance for these skin lesions depends on several factors, including the characteristics of the mole or scar, the depth of the lesion, and the number of treatment sessions.
When it comes to raised moles, CO2 laser therapy can be effective in reducing their height and making them appear flatter. The laser works by vaporizing the outer layers of skin, which can help to remove the raised portion of the mole. However, it is important to note that not all moles will respond the same way to laser treatment. Some moles may be deeper or have a different composition, which can affect how well they flatten out. In some cases, multiple sessions may be required to achieve the desired results. If a mole appears to remain unchanged after several treatments, it may be due to its depth or structure, and further evaluation by a dermatologist is recommended.
Regarding hypertrophic scars, CO2 laser therapy can also be beneficial. These scars are raised and can be red or discolored, often resulting from an injury or surgery. The laser helps to resurface the skin, promoting collagen remodeling and reducing the scar's height and pigmentation. Mature hypertrophic scars tend to respond better to laser treatments compared to newer scars, as they have stabilized over time. However, the degree of flattening can vary, and some patients may require a combination of treatments, including laser therapy, silicone gel sheets, or corticosteroid injections, to achieve optimal results.
It is also essential to consider that CO2 laser treatments can have side effects, such as redness, swelling, and potential changes in pigmentation. These effects are typically temporary, but patients with darker skin tones may be at a higher risk for post-inflammatory hyperpigmentation. Therefore, a thorough consultation with a qualified dermatologist is crucial to assess the suitability of CO2 laser treatment for your specific case.
In your situation, where you have undergone multiple sessions of CO2 laser treatment for a raised mole that has not significantly changed after the third session, it may be worth discussing your concerns with your dermatologist. They can evaluate the mole's characteristics and determine if additional treatments are necessary or if alternative options should be considered. Factors such as the mole's depth, size, and hair growth can influence the treatment outcome, and your dermatologist may recommend a different approach if the current treatment is not yielding satisfactory results.
In summary, CO2 laser treatments can effectively flatten raised moles and hypertrophic scars, but individual responses vary. A comprehensive evaluation by a dermatologist is essential to tailor the treatment plan to your specific needs and to manage expectations regarding the outcomes. If you have further concerns or questions about your treatment, do not hesitate to reach out to your healthcare provider for personalized advice.
