May I ask about the blood test for hCG levels?
Hello: I've seen some people who cannot detect pregnancy in the early stages, and after testing, they find out they are already four to five weeks along.
I'm not sure if they only used home pregnancy tests.
During the time when they couldn't detect pregnancy, would blood tests also not show positive results? Additionally, what is the earliest time after intercourse that a blood test can accurately confirm pregnancy? Thank you.
Wo, 20~29 year old female. Ask Date: 2016/05/02
Dr. Cao Guotao reply Obstetrics and Gynecology
Hello, it is generally recommended to wait at least 12 days after conception to take a pregnancy test.
A home pregnancy test is usually sufficient, and there is no need for a blood test to check hCG levels.
Wishing you good health!

Understanding HCG Blood Tests: When Can You Detect Pregnancy Accurately?
Human Chorionic Gonadotropin (HCG) is a hormone produced during pregnancy, and its presence in the blood or urine is a primary indicator of pregnancy. Blood tests for HCG are generally more sensitive and can detect lower levels of the hormone compared to home pregnancy tests (HPTs) that use urine samples. This sensitivity makes blood tests a reliable method for confirming pregnancy, especially in early stages.
To address your question regarding the detection of pregnancy through blood tests, it is essential to understand the timing and the factors that influence HCG levels. After conception, HCG levels begin to rise, typically doubling every 48 to 72 hours in early pregnancy. However, the timing of ovulation and implantation can vary significantly among individuals, which can affect when HCG becomes detectable.
For most women, HCG can be detected in the blood approximately 6 to 8 days after ovulation and fertilization. This means that if you have had unprotected intercourse, you might consider getting a blood test about a week after your expected ovulation date. However, for the most accurate results, it is often recommended to wait until at least the first day of your missed period, which is usually about two weeks after ovulation. At this point, HCG levels are typically high enough to be detected reliably in a blood test.
In cases where women take a blood test too early, it is possible that the HCG levels are still too low to be detected, leading to a false-negative result. This is why some women may experience a situation where they suspect they are pregnant but receive negative results from both urine and blood tests. If a woman has irregular menstrual cycles or is unsure of her ovulation timing, it may be prudent to wait a little longer before testing or consult with a healthcare provider for guidance.
Regarding the accuracy of blood tests, they are considered highly reliable. A quantitative HCG blood test can measure the exact amount of HCG in the bloodstream, providing more information than a qualitative test, which simply indicates whether HCG is present or not. If the HCG level is below 5 mIU/mL, it is generally interpreted as not being pregnant. Levels between 5 and 25 mIU/mL may require follow-up testing to confirm pregnancy, while levels above 25 mIU/mL are typically considered positive for pregnancy.
In summary, for the most accurate detection of pregnancy through blood tests, it is advisable to wait at least one week after a missed period or about two weeks after potential conception. If you suspect pregnancy but receive negative results, consider retesting a few days later or consulting with a healthcare professional for further evaluation. This approach ensures that you receive the most accurate information regarding your pregnancy status and can help guide any necessary next steps in your reproductive health journey.
