Taking metformin together with the medication for self-enjoyment?

Hello, Doctor.
Last September, I was diagnosed with polycystic ovary syndrome (PCOS).
My doctor recommended that I take birth control pills to improve my condition, and I chose the Yaz birth control pill, which has really helped a lot.
My menstrual cycle has become regular, and the acne I had on my face has disappeared, resulting in much better skin.
However, after taking it for six months, I stopped the medication for 2-3 months, and now the symptoms have returned.
I am starting to get acne again, and my menstrual cycle is irregular.
I was considering continuing with Yaz, but I gained 8 kilograms while on it.
This year, I managed to lose about 4 kilograms through diet control and exercise after stopping the medication.
I was wondering if I could take Metformin, which helps control insulin and manage weight, alongside Yaz? Thank you!

Hello Ting: Thank you for your inquiry.
After reviewing the package insert for the contraceptive pill, I found no interactions with metformin.

Yes, Metformin can be taken with Yaz (the brand name for the oral contraceptive containing drospirenone and ethinyl estradiol) for the management of Polycystic Ovary Syndrome (PCOS). This combination is often considered beneficial for women with PCOS, especially if they are experiencing symptoms such as irregular menstrual cycles, acne, and weight gain.

Metformin is primarily used to improve insulin sensitivity and lower blood sugar levels, which can be particularly helpful for women with PCOS who often have insulin resistance. This medication can aid in weight management, which is crucial since many women with PCOS struggle with weight gain. By improving insulin sensitivity, Metformin may also help regulate menstrual cycles and reduce androgen levels, which can lead to improvements in acne and hirsutism (excess hair growth).

On the other hand, Yaz is a combination oral contraceptive that can help regulate menstrual cycles and reduce symptoms associated with hormonal imbalances, such as acne. The use of Yaz can also provide contraceptive benefits, which may be important for women who are not currently trying to conceive.

The combination of Metformin and Yaz does not typically result in significant drug interactions, making it a safe option for many women with PCOS. However, it is essential to monitor for any side effects or changes in your health status while taking these medications. Common side effects of Metformin include gastrointestinal issues such as nausea, diarrhea, and abdominal discomfort, especially when starting the medication. These side effects can often be mitigated by starting with a lower dose and gradually increasing it.

In your case, since you have experienced weight gain while on Yaz, adding Metformin could potentially help you manage your weight more effectively. It is also important to maintain a healthy lifestyle through diet and exercise, as these factors play a significant role in managing PCOS symptoms.

Before starting any new medication or combination of medications, it is crucial to have a thorough discussion with your healthcare provider. They can provide personalized advice based on your medical history, current health status, and specific symptoms. Additionally, they can help monitor your progress and make any necessary adjustments to your treatment plan.

In summary, taking Metformin alongside Yaz can be an effective strategy for managing PCOS symptoms, particularly for those dealing with insulin resistance and weight management issues. Always consult with your healthcare provider to ensure that this combination is appropriate for your individual circumstances and to discuss any potential side effects or concerns you may have.
