Phimosis issue

Hello doctor, when I am not erect and even after erection, my foreskin covers the glans (about 1/5 of the glans is exposed after erection).
However, I can manually retract the foreskin to the coronal sulcus.
When erect, if I pull the foreskin down to the coronal sulcus, I feel a tight sensation as if it is being pulled by the frenulum, so I am hesitant to apply more force in that state for fear that the frenulum might tear.
Doctor, will circumcision improve this condition? Would you recommend circumcision, or are there other solutions? Also, since I have not had a female partner, I am unsure if this situation affects sexual intercourse.

Mr. Chen, 20~29 year old female. Ask Date: 2016/05/03

Dr. Li Xinyi reply Urology

It sounds like the foreskin issue may require surgical intervention, but it's important to have an in-person evaluation.
It is recommended to visit a urologist for examination and to discuss the matter with the physician.

More Info

Phimosis, a condition where the foreskin cannot be easily retracted over the glans (head) of the penis, can indeed impact sexual health and function. Your description indicates that while you can retract the foreskin when not erect, it becomes tight and difficult to pull back when you are erect, which can lead to discomfort or pain. This situation can affect sexual experiences and may lead to anxiety regarding sexual performance.

Treatment Options for Phimosis
1. Conservative Management: In mild cases of phimosis, conservative treatments may be effective. This can include the use of topical corticosteroids to help soften the foreskin and make it easier to retract over time. Gentle stretching exercises can also be beneficial, but these should be done cautiously to avoid injury.

2. Surgical Options: If conservative measures do not provide relief or if the phimosis is causing significant discomfort, pain, or recurrent infections, surgical intervention may be recommended. The most common surgical procedure is circumcision, where the foreskin is removed entirely. This procedure can alleviate the tightness and prevent future complications. Another option is preputioplasty, which involves widening the opening of the foreskin without complete removal. This can be a less invasive option for some individuals.

3. Consultation with a Specialist: It is advisable to consult a urologist who specializes in male reproductive health. They can provide a thorough examination and discuss the best treatment options tailored to your specific situation. They can also address any concerns you may have about the procedure, including recovery time and potential complications.

Impact on Sexual Health
Regarding your concern about how phimosis might affect sexual activity, it can indeed play a role in sexual health. The tightness of the foreskin during an erection can lead to discomfort, which may affect your sexual experiences. Additionally, if you are apprehensive about the condition, it could lead to anxiety during sexual encounters, which can further impact performance and enjoyment.

While you mentioned not currently having a female partner, it is important to consider that phimosis can affect sexual relationships in the future. If the condition causes pain or discomfort during intercourse, it could lead to avoidance of sexual activity, which may impact intimacy with a partner.

Conclusion
In summary, phimosis can be effectively managed through conservative treatments or surgical options, depending on the severity of the condition and the level of discomfort it causes. Consulting with a urologist is crucial to determine the most appropriate course of action. Addressing this condition not only improves physical comfort but can also enhance your overall sexual health and confidence in future intimate relationships. If you have any further questions or concerns, do not hesitate to reach out to a healthcare professional who can provide personalized advice and support.
