Wrist fracture
I have had a steel screw implanted near my wrist joint for 7 months.
The doctor said it can be surgically removed, but the physical therapist mentioned that the joint may still feel stiff after the screw is taken out.
Is it okay to leave the screw in place? Will removing it improve joint mobility? I used self-funded materials.
Thank you for your guidance!

The removal of intramedullary nails for wrist joint fractures can occur once healing is achieved, and is not related to joint mobility, but rather to the extent and mechanism of the injury.

When it comes to the question of whether to remove steel pins after wrist fracture surgery, there are several factors to consider. The decision to remove or leave the pins in place often depends on the individual patient's circumstances, the type of fracture, the healing process, and any symptoms the patient may be experiencing.
Firstly, it is important to understand that the primary purpose of the steel pins (or screws) used in orthopedic surgery is to stabilize the fracture and allow for proper healing. In many cases, these pins are made from materials such as titanium or stainless steel, which are biocompatible and can remain in the body without causing issues. In fact, many patients live with these implants for years without any problems.
In your case, since it has been seven months since the surgery, it is likely that your wrist has had sufficient time to heal. If your doctor has suggested that the pins can be removed, it may be because they are no longer necessary for the stability of the wrist joint. However, the decision to proceed with removal should be made after considering several factors:
1. Symptoms: If you are experiencing discomfort, pain, or a sensation of the pins causing interference with joint movement, this may warrant removal. On the other hand, if you are not experiencing significant symptoms, it may be reasonable to leave them in place.
2. Joint Function: You mentioned that your physical therapist indicated that removing the pins may not necessarily improve joint function. This is an important consideration. Sometimes, the stiffness or "catching" sensation in the joint may be due to scar tissue or other factors unrelated to the presence of the pins. If the pins are not causing any pain or functional limitations, it may be advisable to leave them in.
3. Risks of Surgery: Any surgical procedure carries risks, including infection, nerve damage, and complications related to anesthesia. If the potential benefits of removing the pins do not outweigh these risks, it may be better to keep them in place.
4. Long-term Considerations: Some patients worry about having foreign objects in their bodies. However, if the pins are not causing any issues, many orthopedic surgeons recommend leaving them in. If you are concerned about future complications, such as arthritis or joint stiffness, discuss these concerns with your surgeon.
5. Follow-up Care: Regardless of whether you choose to have the pins removed or leave them in, ongoing rehabilitation and physical therapy are crucial for regaining full function of the wrist. Your therapist can help you develop a tailored program to address any stiffness or discomfort you may be experiencing.
In summary, the decision to remove steel pins after wrist fracture surgery should be made collaboratively with your orthopedic surgeon, taking into account your symptoms, the condition of your wrist, and the potential risks and benefits of surgery. If you are not experiencing significant issues and your doctor believes the pins can remain, it may be perfectly acceptable to leave them in place. Always prioritize open communication with your healthcare team to ensure the best outcome for your recovery.
