Rapid heartbeat?
Dear Dr.
He,
Thank you very much for your response.
Regarding the question I asked last time about my heart rate suddenly jumping to over 150 beats per minute, what kind of tests should I undergo to identify the cause? This situation is very distressing for me.
Sometimes when my heart races, I immediately ride my bike to the emergency room, and by the time I arrive, my heart rate has already dropped to around 130 beats per minute.
The doctor said there was nothing abnormal.
Additionally, I have recently experienced episodes where I suddenly wake up from sleep due to palpitations, and my pulse fluctuates rapidly and then slows down, with the fast heart rate lasting for a few beats before it becomes very slow.
This lasts for about ten minutes, and I feel quite anxious.
Could you please explain what symptoms this might indicate? Is it serious? Previously, my heart rate would be fast but regular, and I have not experienced this kind of irregularity (it has only happened twice and for a short duration).
What could be the cause of this? Thank you for your guidance.

Hello, you can undergo a 24-hour Holter monitor or even a week-long ECG monitoring to determine if there is indeed an arrhythmia or the supraventricular tachycardia that you are concerned about.
It is advisable to visit a cardiology clinic for further evaluation.

Irregular heartbeats, also known as arrhythmias, can be concerning and may arise from various causes. The symptoms you describe—sudden increases in heart rate, waking up with palpitations, and fluctuations between fast and slow heartbeats—are indeed alarming and warrant further investigation.
Firstly, it’s important to understand that the heart has an electrical system that controls the rhythm of your heartbeat. When this system is disrupted, it can lead to irregular heartbeats. Common causes of arrhythmias include stress, anxiety, caffeine, alcohol, certain medications, and underlying health conditions such as heart disease, thyroid problems, or electrolyte imbalances. Given your experiences, it’s possible that stress or anxiety could be contributing factors, especially if you have been feeling overwhelmed or anxious lately.
In your case, the episodes of your heart rate spiking to 150 beats per minute and then returning to a lower rate could indicate a type of arrhythmia known as paroxysmal supraventricular tachycardia (PSVT), where the heart suddenly beats very fast for a short period. This can happen in response to triggers like stress or caffeine and is often not dangerous but can be uncomfortable. However, the fact that you also experience fluctuations in your heart rate—where it goes from fast to slow—could suggest other types of arrhythmias, such as atrial fibrillation or other forms of atrial flutter.
To diagnose the underlying cause of your symptoms, several tests may be recommended. An electrocardiogram (ECG or EKG) is a primary tool that can help identify arrhythmias when they occur. However, since your episodes are intermittent, a 24-hour Holter monitor may be more effective. This device records your heart's rhythm continuously over a day or more, capturing any irregularities that occur during your normal activities. Additionally, an event monitor, which you wear for a longer period, can be triggered by you when you feel symptoms, allowing for a more targeted analysis.
If your symptoms persist or worsen, it’s crucial to seek medical attention. You should also consider discussing your experiences with a cardiologist, who can provide a comprehensive evaluation and possibly recommend further testing, such as an echocardiogram to assess the structure of your heart or blood tests to check for electrolyte imbalances or thyroid function.
In the meantime, managing stress through relaxation techniques, regular exercise, and a healthy diet can be beneficial. Avoiding stimulants like caffeine and alcohol may also help reduce the frequency of your symptoms. Keeping a diary of your episodes, noting when they occur, their duration, and any associated activities or stressors, can provide valuable information for your healthcare provider.
In summary, while occasional irregular heartbeats can be benign, your symptoms warrant further investigation to rule out any serious underlying conditions. Don’t hesitate to reach out to a healthcare professional to discuss your symptoms and get the appropriate care. Your heart health is paramount, and understanding the causes of your irregular heartbeats is the first step toward effective management.
