Coughing up blood-tinged phlegm in the morning?

About 20 days ago, I had a cold that hasn't completely resolved.
In the past two days, I've noticed blood in my sputum when I wake up in the morning.
The first sputum was all blood, the second had less, and by the third, there was none.
This only occurs in the morning; at other times of the day, there is no blood.
What could be the cause?

Mr. Chen, 40~49 year old female. Ask Date: 2016/05/15

Dr. Zheng Jueyi reply Otolaryngology

Hello: If there is blood in the sputum, it is advisable to seek medical examination, as it may be related to bleeding from the nasal mucosa.
However, other tumor-related issues must also be ruled out.
Wishing you peace and good health.
Sincerely, Dr.
Cheng Jue-Yi, Director of the Department of Otolaryngology, Taoyuan Hospital, Ministry of Health and Welfare.

Experiencing blood in your phlegm, especially after a recent cold, can be concerning and warrants careful consideration. The presence of blood in your morning phlegm could be attributed to several factors, particularly following a respiratory infection like a cold.
Firstly, during a cold, the respiratory tract becomes inflamed, and the mucous membranes in the nose and throat can become irritated. This irritation can lead to small blood vessels breaking, especially when you cough or clear your throat. The act of coughing can create enough pressure to rupture these tiny vessels, resulting in blood being mixed with phlegm. This is often more pronounced in the morning because mucus can accumulate overnight, and the first cough of the day may dislodge this mucus, revealing any blood that has mixed in.

Another potential cause is post-nasal drip, which occurs when excess mucus from the nasal passages drips down the back of the throat. This can lead to throat irritation and coughing, which may also cause minor bleeding. If you have been blowing your nose frequently due to congestion, this can further exacerbate the irritation and lead to bleeding.

Additionally, environmental factors such as dry air, especially during winter months, can dry out the nasal passages and throat, making them more susceptible to bleeding. If you live in an area with low humidity or have been using heating systems that dry out the air, this could contribute to the problem.

While occasional blood in phlegm can be benign, it is crucial to monitor the situation. If the bleeding persists, increases in frequency, or is accompanied by other symptoms such as fever, significant pain, or difficulty breathing, it is essential to seek medical attention. A healthcare provider may perform a thorough examination, which could include a nasal endoscopy or imaging studies, to rule out more serious conditions such as sinusitis, bronchitis, or even more severe issues like lung infections or tumors.

In summary, while the presence of blood in your phlegm after a cold can often be attributed to irritation and inflammation of the respiratory tract, it is important to remain vigilant. If the symptoms do not resolve or worsen, consulting with a healthcare professional is advisable to ensure proper diagnosis and treatment. Maintaining hydration, using a humidifier, and avoiding irritants such as smoke can also help alleviate symptoms and promote healing.
