Prolonged menstruation

The menstrual cycle is approximately 30 days, but occasionally it can extend to 35-40 days (about once every six months).
Each period lasts for 7 days and is usually completely cleared.
This time, my period arrived after 40 days, and the blood flow was noticeably heavier.
Starting from the fourth day, I began to notice brown discharge, and today is already the tenth day, yet I still have a small amount of brown discharge.
Could this be related to the traditional Chinese medicine I took for my allergic constitution, or is there really an issue with my uterus? Thank you!

Dr. Lin Manying reply Obstetrics and Gynecology

The menstrual cycle and its duration can sometimes be affected by medication, sleep, and stress.
If it persists for more than 10 days, it is advisable to seek medical evaluation.

Understanding prolonged menstrual cycles can be complex, as various factors can influence menstrual regularity and flow. A typical menstrual cycle ranges from 21 to 35 days, with an average duration of about 28 days. However, variations can occur due to numerous reasons, including hormonal imbalances, lifestyle factors, and underlying medical conditions.

In your case, experiencing menstrual cycles that extend to 35-40 days occasionally, along with a heavier flow and the presence of brown discharge, raises several considerations. Brown discharge typically indicates older blood that has taken longer to exit the uterus, which can be common at the beginning or end of a menstrual period. However, if this occurs outside of your typical cycle, it may warrant further investigation.

1. Hormonal Imbalances: One of the most common causes of irregular menstrual cycles is hormonal imbalance. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or changes in estrogen and progesterone levels can lead to prolonged cycles and variations in flow. If you have experienced significant stress, weight changes, or lifestyle alterations, these could also impact your hormonal balance.

2. Uterine Conditions: Conditions such as fibroids, polyps, or endometriosis can cause changes in menstrual patterns. These conditions can lead to heavier bleeding, prolonged periods, and additional symptoms such as pelvic pain. If you have a family history of these conditions or if you experience significant discomfort, it may be worth discussing with your healthcare provider.

3. Medications and Herbal Remedies: You mentioned taking traditional Chinese medicine for allergy management. Some herbal remedies can influence menstrual cycles, either by affecting hormonal levels or by having a direct impact on uterine health. It’s crucial to inform your healthcare provider about any medications or supplements you are taking, as they can help determine if these might be contributing to your symptoms.

4. Age and Menstrual Changes: If you are in your late 30s or early 40s, it’s important to consider that perimenopause can begin several years before menopause. This transitional phase can lead to irregular cycles, heavier or lighter periods, and other symptoms due to fluctuating hormone levels.

5. Underlying Health Issues: Conditions such as diabetes, obesity, or chronic illnesses can also affect menstrual cycles. If you have other symptoms or health concerns, these should be evaluated in conjunction with your menstrual irregularities.

Given your situation, it would be advisable to consult with a healthcare provider, preferably a gynecologist, who can perform a thorough evaluation. This may include a physical examination, blood tests to check hormone levels, and possibly imaging studies like an ultrasound to assess the uterus and ovaries.
In summary, while occasional variations in menstrual cycles can be normal, persistent changes, especially those accompanied by other symptoms, should be evaluated. Understanding the underlying causes can help in managing your menstrual health effectively. It’s essential to maintain open communication with your healthcare provider about your symptoms and any treatments you are undergoing, as this will aid in providing you with the best care possible.
