Do I have a chance of getting pregnant?

I have polycystic ovary syndrome (PCOS), and my menstrual cycle is very irregular.
Recently, my period finally arrived, but it only lasted for one day for two consecutive months.
I have also been diagnosed with infertility, as my sperm viability is only 40%.
We often engage in unprotected intercourse with ejaculation outside, but pregnancy tests consistently show only one line.
My question is, do we still have a chance of getting pregnant? Additionally, could having a period that only lasts one day and then stops be a sign of early menopause?

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, the general definition of infertility is the inability to conceive after one year of regular sexual intercourse.
In cases of infertility, both partners should undergo examinations at a gynecology clinic, including semen analysis, hysterosalpingography, ovulation testing, and hormone blood tests, among others.
Since you are only 19 years old and practicing withdrawal, you may not be considered infertile and can simply observe for now.
If you still have concerns, you may consult your regular gynecologist.
Wishing you good health.

Polycystic Ovary Syndrome (PCOS) is a common endocrine disorder that can significantly affect a woman's menstrual cycle, fertility, and overall reproductive health. Women with PCOS often experience irregular periods, which can make it challenging to predict ovulation and conceive. However, it is still possible to get pregnant with PCOS, even if you have irregular periods.

In your case, it seems that you have been experiencing very short menstrual cycles, with periods lasting only one day. This could indicate a hormonal imbalance, which is common in women with PCOS. Irregular or very short menstrual cycles can make it difficult to track ovulation, which is crucial for conception. Typically, ovulation occurs about 14 days before the start of your next period, but with irregular cycles, this can be unpredictable.

Despite these challenges, many women with PCOS can conceive naturally or with the help of fertility treatments. The key is to manage the symptoms of PCOS effectively. This may include lifestyle changes such as maintaining a healthy weight, regular exercise, and a balanced diet. Weight loss can help regulate menstrual cycles and improve insulin sensitivity, which is often an issue for women with PCOS.

Regarding your concern about the possibility of early menopause, it is important to note that while PCOS can lead to irregular cycles, it does not necessarily mean you are approaching menopause. Early menopause is typically defined as menopause occurring before the age of 40, and it is characterized by the cessation of menstruation for 12 consecutive months. If you are still experiencing some menstrual bleeding, even if it is irregular, it is unlikely that you are in menopause. However, it is essential to consult with a healthcare provider to evaluate your symptoms and discuss any concerns you may have about your reproductive health.

As for your question about the likelihood of pregnancy, even with irregular periods and a diagnosis of infertility, there is still a chance of conception. The fact that you are engaging in unprotected intercourse increases the likelihood of pregnancy, but the irregularity of your cycles may make it more challenging to conceive. It is advisable to track your menstrual cycles and consider using ovulation predictor kits to help identify your fertile window.

If you have been trying to conceive for an extended period without success, it may be beneficial to seek assistance from a fertility specialist. They can provide a comprehensive evaluation of both partners, including hormone testing and semen analysis, to determine the best course of action. Treatments for PCOS-related infertility may include medications to induce ovulation, such as Clomiphene citrate or letrozole, or assisted reproductive technologies like in vitro fertilization (IVF).

In summary, while PCOS and irregular periods can complicate conception, it is still possible to get pregnant. Monitoring your menstrual cycles, maintaining a healthy lifestyle, and seeking medical advice can significantly improve your chances of conception. If you have concerns about your menstrual cycle or fertility, it is always best to consult with a healthcare professional who can provide personalized guidance and support.
