Rash on the glans penis?
Hello Dr.
Song, recently I experienced two episodes of nocturnal emissions within a week.
During the first occurrence, I got up to clean myself, but during the second, I slept until morning.
When I woke up, I found that the semen had dried, and there was a dry, wrinkled patch on my underwear where there was no wet or sticky semen.
However, the glans of my penis felt a slight pain, similar to the sensation of a mosquito bite.
A few days later, while showering, I noticed three or four small bumps appearing on the glans, but they were neither painful nor itchy.
At the same time, I have been experiencing itching in the groin area, for which I have been applying ointment.
Since I have not had sexual relations with anyone for nearly five years, I believe it is unlikely to be a sexually transmitted infection.
Today, while showering, I noticed that the bumps have decreased in size but seem to have increased in number.
I can't shake the feeling that the glans looks unusual.
What could be the cause of this? I am unsure if the nocturnal emissions, groin itching, and the bumps on the glans are related, but they did occur in succession.
I kindly ask for your explanation.
Thank you.

Itchy groin is mostly caused by tinea cruris, and moisture in the genital area (such as not cleaning up after nocturnal emissions) is also one of the reasons.
Tinea cruris can easily recur and may not heal.
If there are small bumps on the glans and there has been no inappropriate sexual contact, it is likely due to phimosis and insufficient cleaning, leading to chronic infection or fungal growth.
Kenacomb can be used for treatment.

More Info
Understanding the symptoms you've described—dream emissions (nocturnal emissions), discomfort in the glans (the head of the penis), and the appearance of small bumps—can be complex, as they may stem from various causes. It’s essential to approach this with a comprehensive understanding of potential medical conditions and their interrelations.
Nocturnal Emissions
Nocturnal emissions, commonly known as "wet dreams," are a normal physiological occurrence, especially in younger males. They happen when the body releases semen during sleep, often accompanied by erotic dreams. Experiencing these is not unusual and can happen sporadically, even in individuals who are not sexually active. The fact that you had two occurrences within a week is not alarming unless accompanied by other concerning symptoms.
Discomfort in the Glans
The sensation you described—similar to a mosquito bite—could be attributed to several factors. It might be a mild irritation or inflammation, possibly due to friction, hygiene practices, or even an allergic reaction to soaps or other products used in the genital area.
Rash on the Glans
The appearance of small bumps on the glans can be concerning, but it’s important to note that not all bumps or rashes are indicative of sexually transmitted infections (STIs). Given your history of not being sexually active for five years, the likelihood of an STI is reduced, but it cannot be entirely ruled out without proper medical evaluation.
Common causes for bumps on the glans include:
- Pearly Penile Papules: These are small, harmless bumps that are often mistaken for warts or other infections.
- Folliculitis: Inflammation of hair follicles can lead to small, red bumps.
- Contact Dermatitis: An allergic reaction to products like soaps, lotions, or even laundry detergents can cause irritation and rashes.
- Balanitis: Inflammation of the glans, often due to poor hygiene or infections, can lead to discomfort and rashes.
Itching in the Groin Area
The itching in your groin could be related to a fungal infection, such as jock itch, which is common in warm, moist areas of the body. It could also be a result of irritation from sweat or tight clothing.
Recommendations
1. Hygiene: Maintain good hygiene by washing the genital area with mild soap and water. Ensure the area is dry after washing, as moisture can promote fungal growth.
2. Avoid Irritants: If you suspect that soaps or lotions may be causing irritation, consider switching to hypoallergenic products.
3. Monitor Symptoms: Keep an eye on the bumps and any changes in symptoms. If they persist, worsen, or if you develop additional symptoms (such as pain, discharge, or fever), it’s crucial to seek medical attention.
4. Consult a Healthcare Provider: Given the combination of symptoms, it would be wise to consult a healthcare provider, preferably a urologist or dermatologist. They can perform a physical examination, possibly conduct tests (like swabs or cultures), and provide a definitive diagnosis.
5. Avoid Self-Diagnosis: While online resources can provide general information, they cannot replace professional medical advice. It’s essential to get a proper evaluation to rule out any serious conditions.
In conclusion, while your symptoms may not necessarily indicate a severe issue, they warrant attention to ensure proper care and treatment. It’s always best to err on the side of caution when it comes to changes in your health, especially in sensitive areas.
