Pregnancy immediately after tubal ligation?

On April 18, 2016, a right salpingectomy was performed due to an ectopic pregnancy.
Intercourse occurred on April 28 and 30, and menstruation has not yet occurred.
Is it possible to be pregnant again? If not pregnant, how long after the procedure can menstruation be expected to return?

Qian, 30~39 year old female. Ask Date: 2016/05/24

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, based on your description, the chances of pregnancy are not very high.
I recommend that you take appropriate contraceptive measures.
If your period has not returned after one and a half months post-surgery, it is still necessary to take a urine test to check for pregnancy.
Thank you! Wishing you safety and health.

More Info

After a tubal removal, particularly following a procedure like a salpingectomy (removal of a fallopian tube), the possibility of getting pregnant depends on several factors, including the remaining reproductive anatomy and the timing of ovulation. In your case, since you had a right-sided ectopic pregnancy and the right fallopian tube was removed, you still have the left fallopian tube intact. This means that if you are ovulating from the left ovary, there is still a chance for sperm to meet the egg and result in pregnancy.

Given that you had intercourse shortly after the surgery, it is indeed possible that you could be pregnant. However, the timing of ovulation is crucial. Typically, ovulation occurs about 14 days before the start of your next menstrual period. If you had your surgery on April 18 and had intercourse on April 28 and 30, you may have been in your fertile window, depending on your cycle length. If you do not get your period within a few weeks after your last intercourse, it would be advisable to take a home pregnancy test to check for pregnancy.

If the test is negative and you still do not get your period, it could be due to several reasons, including hormonal changes following the surgery, stress, or other factors affecting your menstrual cycle. It is not uncommon for menstrual cycles to be irregular after a surgical procedure, and it may take some time for your body to return to its normal rhythm.

If you do not have a period within 4-6 weeks post-surgery, or if you have any concerning symptoms such as severe pain, unusual discharge, or fever, it is important to consult your healthcare provider. They may want to perform an ultrasound or other tests to ensure that everything is healing properly and to rule out any complications.

In summary, yes, it is possible to get pregnant after a tubal removal, especially if the remaining fallopian tube is functional and you are ovulating. If you suspect you might be pregnant, taking a pregnancy test after a missed period is a good next step. If you have concerns about your menstrual cycle or potential pregnancy, reaching out to your healthcare provider for personalized advice is always recommended.
