Loss of appetite and upper abdominal bloating?
Hello Doctor: I’m not sure if it’s because of the hot weather lately, but for over a week now, I haven’t had much of an appetite, and my upper abdomen is quite distended (which is unusual for me).
It feels so bloated that I can hardly sit comfortably, but even though I’m bloated, I haven’t been burping or passing gas.
I thought I might have gained weight, but my weight hasn’t changed much (in fact, it seems to have decreased a bit).
I usually don’t mind the heat, but these past few days, I’ve felt extremely hot, and my skin feels warm to the touch.
Is this indigestion, or could it be heat exhaustion?
Additionally, I have another issue that has been ongoing for a while: I experience pain when wiping after using the bathroom (it feels like a sore spot, and there’s a little blood, but not a lot, just a few drops).
Could this be hemorrhoids, or is it just a minor infection? I have irregular menstrual cycles, and I tend to use sanitary products for extended periods, which might contribute to a lack of airflow.
Could my recent loss of appetite and upper abdominal bloating be related to this, or is it more likely a gastrointestinal issue? (I do not have any constipation problems.) Thank you!

Hello, Miss.
① You may have a gastrointestinal motility disorder, and it would be advisable to visit a gastroenterology clinic for evaluation.
② Additionally, it would be beneficial to assess any discomfort in the anal area, which is likely related to internal or external hemorrhoids.

More Info
Loss of appetite and abdominal bloating can be concerning symptoms, especially when they persist for over a week. Based on your description, there are several potential causes and solutions to consider.
Possible Causes
1. Gastrointestinal Issues:
- Dyspepsia: This is a common condition that can cause bloating, discomfort, and loss of appetite. It may be related to overeating, eating too quickly, or consuming rich or fatty foods.
- Gastritis: Inflammation of the stomach lining can lead to similar symptoms, often exacerbated by stress or certain medications (like NSAIDs).
- Irritable Bowel Syndrome (IBS): This functional gastrointestinal disorder can cause bloating, abdominal pain, and changes in bowel habits. Stress and certain foods can trigger symptoms.
2. Heat and Dehydration:
- Hot weather can affect your appetite and digestion. Dehydration can lead to a feeling of fullness and discomfort. It’s essential to stay hydrated, especially in hot weather.
3. Infections:
- Gastroenteritis or other infections can lead to loss of appetite and bloating. Even if you don’t have diarrhea, a mild infection can cause these symptoms.
4. Hormonal Changes:
- If your menstrual cycle is irregular, hormonal fluctuations can affect your appetite and digestion. This could also explain the discomfort when wiping, possibly related to hemorrhoids or irritation.
5. Psychological Factors:
- Stress and anxiety can significantly impact appetite and digestion. If you’re feeling overwhelmed, it might manifest physically.
Solutions
1. Dietary Adjustments:
- Try eating smaller, more frequent meals instead of large ones. Focus on easily digestible foods, such as bananas, rice, applesauce, and toast (the BRAT diet).
- Avoid carbonated beverages, high-fat foods, and excessive fiber until symptoms improve.
2. Hydration:
- Ensure you’re drinking enough fluids, especially water. Herbal teas can also be soothing for the digestive system.
3. Rest and Relaxation:
- Stress management techniques, such as deep breathing, meditation, or yoga, can help alleviate symptoms related to anxiety and stress.
4. Medical Evaluation:
- If symptoms persist, it’s crucial to consult a healthcare professional. They may recommend tests to rule out any underlying conditions, such as gastrointestinal disorders or infections.
5. Addressing Rectal Pain:
- The pain you experience when wiping, especially if there is blood, could indicate hemorrhoids or anal fissures. Maintaining good hygiene, using soft toilet paper, and applying topical treatments can help. If the pain persists or worsens, seek medical advice.
Conclusion
In summary, loss of appetite and abdominal bloating can stem from various causes, including gastrointestinal issues, heat-related factors, hormonal changes, and psychological stress. It’s essential to monitor your symptoms and consult a healthcare professional if they persist or worsen. Addressing the rectal pain is also important, as it could be related to hemorrhoids or other conditions. Taking a proactive approach to your health will help you find relief and improve your overall well-being.
