Continuous shedding of oral mucosa?

Doctor, a certain area of the oral mucosa on my right cheek is wrinkled and keeps peeling off.
However, I don't notice any significant color changes in that area.
When I touch it with my tongue, it feels wrinkled.
Could you please explain why this is happening? Thank you.

Jean, 20~29 year old female. Ask Date: 2016/06/02

Dr. Zeng Guanghui reply Dentistry

The above situation may be caused by an immune system interference leading to an inflammatory response of the oral mucosa.
It is recommended to consult a dentist for further diagnosis and treatment.

Persistent shedding of the oral mucosa can be a concerning symptom, and it is essential to understand the potential causes and solutions. The oral mucosa is the moist tissue lining the mouth, and it can undergo changes due to various factors, including trauma, infections, systemic diseases, and irritants.

Possible Causes of Oral Mucosa Shedding
1. Trauma or Irritation: One of the most common reasons for changes in the oral mucosa is physical trauma. This could be due to aggressive brushing, biting the inside of the cheek, or dental appliances like braces or dentures that may rub against the mucosa. If you have a habit of chewing on your cheeks or lips, this could also lead to irritation and shedding.

2. Infections: Fungal infections, such as oral thrush (caused by Candida), can lead to changes in the mucosa. This condition often presents as white patches but can also cause redness and irritation. Viral infections, like herpes simplex, can also cause lesions that may lead to shedding of the mucosal layer.

3. Systemic Conditions: Certain systemic diseases can manifest in the oral cavity. Conditions such as autoimmune diseases (like lupus or pemphigus vulgaris) can cause the mucosa to become inflamed and shed. Additionally, vitamin deficiencies (especially B vitamins) can lead to changes in the mucosa.

4. Allergic Reactions: Allergies to certain foods, dental materials, or oral hygiene products can lead to localized reactions in the oral mucosa, resulting in irritation and shedding.

5. Hormonal Changes: Hormonal fluctuations, such as those occurring during pregnancy or menopause, can affect the oral mucosa, leading to changes in texture and sensitivity.

Solutions and Recommendations
1. Consult a Specialist: Given the persistence of your symptoms, it is advisable to consult an oral specialist or a dentist who can perform a thorough examination. They may recommend a biopsy if there are any suspicious areas or if the shedding does not resolve with conservative measures.

2. Maintain Oral Hygiene: Ensure that you are practicing good oral hygiene. Use a soft-bristled toothbrush and avoid harsh mouthwashes that may irritate the mucosa. Regular dental check-ups are essential to monitor your oral health.

3. Avoid Irritants: Identify and avoid any potential irritants, such as spicy foods, tobacco, and alcohol, which can exacerbate mucosal irritation.

4. Nutritional Assessment: Consider a nutritional assessment to rule out deficiencies that may contribute to mucosal changes. A balanced diet rich in vitamins and minerals is crucial for maintaining healthy mucosal tissue.

5. Topical Treatments: Depending on the diagnosis, your dentist may recommend topical treatments to soothe the mucosa. These could include corticosteroid ointments or mouth rinses designed to reduce inflammation and promote healing.

6. Stress Management: If stress is a contributing factor, consider stress management techniques such as mindfulness, yoga, or counseling. Stress can exacerbate oral health issues, including habits like cheek biting.

7. Follow-Up: Keep track of your symptoms and follow up with your healthcare provider if there is no improvement or if new symptoms arise. Persistent changes in the oral mucosa should not be ignored, as they can sometimes indicate underlying health issues.

In conclusion, while the shedding of the oral mucosa can be benign, it is essential to investigate the underlying causes, especially if the symptoms persist. A comprehensive approach involving dental care, lifestyle modifications, and possibly medical intervention will help address the issue effectively.
