Menstrual discharge

Hello doctor, I started noticing brown discharge last week, and I'm not sure if it's discharge or menstrual blood.
Sometimes there's a noticeable amount, while other times it appears like flakes in my urine.
Occasionally, I feel a bit of pain at the vaginal opening.
This has been happening for about four to five days, and recently I've seen red menstrual blood, but it only appears during bowel movements or urination; at other times, the pad still has brown discharge.
I'm not quite sure what this situation is.
Thank you.

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the factors that can affect the menstrual cycle or cause bleeding outside of menstruation are numerous, including environment, lifestyle, stress, emotional instability, diet, age, lesions in the uterus or cervix, and pregnancy, among others.
It is advisable to consult a gynecologist if symptoms persist or worsen to determine if there is a hormonal imbalance or other underlying causes.
Thank you! Wishing you peace and health, Dr.
Zhang.

More Info

Brown discharge can be a concerning symptom for many individuals, and understanding its potential causes is crucial for determining whether medical attention is needed. The brown color typically indicates older blood that has taken longer to exit the body, which can occur for various reasons.

Possible Causes of Brown Discharge
1. Menstrual Cycle Variations: It is common for women to experience brown discharge at the beginning or end of their menstrual cycle. This can be old blood that is being expelled as the uterus sheds its lining. If you are nearing your period or just finishing one, this could be a normal occurrence.

2. Ovulation: Some women experience spotting during ovulation, which can sometimes appear brown. This is due to hormonal changes and the release of an egg from the ovary.

3. Pregnancy: Brown discharge can also occur in early pregnancy, often referred to as implantation bleeding. This happens when a fertilized egg attaches to the lining of the uterus. If you suspect you might be pregnant, it’s advisable to take a pregnancy test.

4. Infections: Infections of the vagina or cervix, such as bacterial vaginosis or sexually transmitted infections (STIs), can lead to abnormal discharge, including brown discharge. If accompanied by a foul odor, itching, or unusual discomfort, this could indicate an infection that requires treatment.

5. Hormonal Imbalances: Conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders can lead to irregular menstrual cycles and abnormal discharge. Hormonal fluctuations can cause the body to shed the uterine lining irregularly.

6. Endometriosis or Uterine Fibroids: These conditions can cause abnormal bleeding and discharge, including brown discharge. They may also be associated with pelvic pain, which you mentioned experiencing.

7. Cervical or Uterine Polyps: These benign growths can cause irregular bleeding and discharge. If they are present, they may lead to spotting between periods.

When to Seek Medical Help
Given that you have experienced brown discharge for several days, followed by red blood during urination or bowel movements, it is important to consult a healthcare professional. Here are specific situations where you should seek medical attention:
- Persistent Symptoms: If the brown discharge continues for more than a few days without improvement or if it becomes heavier.

- Pain: If you experience significant pain, especially if it is localized in the lower abdomen or pelvis.

- Unusual Odor: If the discharge has a foul smell, which could indicate an infection.

- Accompanying Symptoms: If you have other symptoms such as fever, chills, or unusual fatigue, these could indicate a more serious condition.

- Changes in Menstrual Cycle: If you notice significant changes in your menstrual cycle, such as missed periods or unusually heavy bleeding.

Conclusion
While brown discharge can often be a normal part of the menstrual cycle, the symptoms you describe warrant further investigation. It is essential to monitor your symptoms closely and consult with a healthcare provider who can perform a thorough evaluation, including a physical examination and possibly some tests, to determine the underlying cause. Early diagnosis and treatment can help manage any potential issues effectively and ensure your reproductive health is maintained.
