Irregular menstruation with postcoital bleeding?

Hello doctor, my period was delayed by two weeks last time, and it finally arrived on May 23 after taking traditional Chinese medicine (the first three days were dark to deep brown with light flow, and only the last one or two days had bright red).
Before this, my menstrual cycle was regular, approximately every 30 days.
On June 9, I had protected sexual intercourse (without ejaculation) after not having intercourse for four years, and it was quite painful.
Since then, I have occasionally noticed brown discharge (cheese-like chunks) when using the restroom, and now I sometimes feel a cramping sensation in my lower abdomen similar to menstrual cramps.
I'm not sure what the issue might be.
Thank you!

Maggie, 20~29 year old female. Ask Date: 2016/06/12

Post-coital bleeding may be due to cervical issues or trauma causing the endometrium to shed.
If it persists for more than a week, it is advisable to seek medical evaluation.

Understanding irregular periods and post-sexual bleeding can be quite complex, as various factors can contribute to these symptoms. Based on your description, it appears that you have experienced a few significant events that may be related to your menstrual cycle and sexual activity.

Firstly, it's important to note that irregular periods can be caused by a variety of factors, including hormonal imbalances, stress, changes in weight, and underlying health conditions. In your case, you mentioned that your last menstrual period was delayed by two weeks and that you experienced a range of colors in your menstrual flow, from dark brown to bright red. This variation in color can often be attributed to the age of the blood; older blood tends to appear darker, while fresh blood is typically bright red. The presence of dark or brown blood at the beginning or end of your period is generally considered normal.

You also mentioned having sexual intercourse on June 9th, which was painful for you. Pain during intercourse, known as dyspareunia, can be caused by several factors, including insufficient lubrication, anxiety, or underlying conditions such as vaginismus or pelvic inflammatory disease. The use of condoms and lubrication is a good practice to prevent injury and enhance comfort, but if pain persists, it may be worth consulting a healthcare provider for further evaluation.

The occurrence of brown discharge or "cottage cheese-like" clumps after sexual activity could indicate several possibilities. One common cause of brown discharge is the presence of old blood, which can occur after menstruation or due to ovulation. However, if the discharge is accompanied by discomfort or unusual odor, it could suggest an infection, such as bacterial vaginosis or a yeast infection. These infections can lead to changes in discharge consistency and color, as well as irritation.

Given that you have experienced these symptoms following sexual activity, it is essential to monitor your condition closely. If the brown discharge continues or if you experience any additional symptoms such as fever, severe pain, or an unusual odor, it would be advisable to seek medical attention. A healthcare provider can perform a pelvic examination and possibly conduct tests to rule out infections or other underlying issues.

In terms of how long post-sexual bleeding or irregular bleeding should be monitored before seeking help, a general guideline is to consult a healthcare provider if bleeding persists for more than three days, especially if it is accompanied by pain or other concerning symptoms. Irregular bleeding can sometimes be a sign of hormonal fluctuations, especially if you have a history of irregular periods, but it is always best to err on the side of caution.

In summary, while some irregularities in your menstrual cycle and post-sexual bleeding can be normal, it is crucial to pay attention to your body and seek medical advice if you have concerns. Regular check-ups with a gynecologist can help you maintain reproductive health and address any issues that may arise.
