Post-Surgery Scarring: Addressing Concerns After Gynecomastia Surgery - Plastic Surgery

Postoperative scars

I just had surgery for gynecomastia, and because the sutured wound is located around the areola, my nipple appears slightly retracted and droopy.
Since only the left nipple has changed like this while the right side looks normal, it looks quite odd.
What can be done to remedy this? Also, could this condition be related to the surgeon's suturing technique?

Xiao Wu, 30~39 year old female. Ask Date: 2016/06/10

Dr. Lu Daokuan reply Plastic Surgery

If there is still unevenness after approximately six months, you may request the original surgeon to perform a minor corrective procedure.

After undergoing gynecomastia surgery, it is not uncommon for patients to have concerns about scarring and the appearance of the nipples. The situation you describe, where one nipple appears to be retracted or drooping while the other remains normal, can be distressing. Here are some insights and recommendations regarding your concerns.

Understanding Post-Surgery Changes
1. Healing Process: After any surgical procedure, including gynecomastia surgery, the body goes through a healing process. This can involve swelling, bruising, and changes in the appearance of the surgical site. It is important to give your body adequate time to heal, which can take several weeks to months. During this time, the appearance of the nipples may change as swelling subsides and the tissues adjust.

2. Scarring and Surgical Technique: The technique used by the surgeon can indeed influence the final appearance of the scars and the surrounding tissues. If the incision was made around the areola (the dark area surrounding the nipple), there may be a risk of changes in nipple position or shape, especially if the tissue is tight or if there is uneven tension during the healing process. However, it is essential to remember that variations in healing can occur, and not all outcomes are solely dependent on surgical technique.

3. Asymmetry: It is also important to note that some degree of asymmetry is common in the human body. After surgery, it may take time for the tissues to settle and for any swelling to resolve, which can help in achieving a more symmetrical appearance.

Recommendations for Addressing Concerns
1. Follow-Up with Your Surgeon: The first step you should take is to schedule a follow-up appointment with your surgeon. They can assess the healing process and determine if the changes you are experiencing are within the normal range. They can also provide guidance on what to expect as healing progresses.

2. Scar Management: If scarring is a concern, your surgeon may recommend treatments to help minimize the appearance of scars. This can include silicone gel sheets, topical treatments, or even laser therapy in some cases. Discuss these options with your healthcare provider.

3. Physical Therapy: In some cases, physical therapy may help improve the appearance of the area and assist in the healing process. A physical therapist with experience in post-surgical recovery can provide exercises and techniques to help with tissue mobility and reduce any tightness.

4. Patience and Observation: Healing takes time, and it is crucial to be patient. Monitor the changes in your body over the next few months. If the situation does not improve or worsens, return to your surgeon for further evaluation.

5. Psychological Support: If you find that your concerns about the appearance of your nipples are affecting your mental well-being, consider speaking with a counselor or therapist. They can provide support and coping strategies as you navigate your recovery.

Conclusion
In summary, while it is understandable to be concerned about the appearance of your nipples after gynecomastia surgery, many factors can influence healing and the final outcome. It is essential to maintain open communication with your surgeon and follow their recommendations for follow-up care. With time and appropriate management, many patients find that their concerns diminish as they continue to heal. Remember, you are not alone in this journey, and support is available to help you through your recovery.
