Is it obesity or gynecomastia?
Hello, doctor.
I would like to ask about a concern I have.
During my developmental years as a student, I experienced swelling and pain in my nipples.
At that time, I would intentionally pinch my nipples, which has resulted in my nipples and areolas being protruded now.
I am approximately 165 cm tall and weigh around 70 kg, which may be considered overweight.
As a result, there is some sagging skin around the protruding nipples and areolas.
Is this related to obesity, or does the protrusion of the nipples and areolas indicate gynecomastia? Honestly, I often hear people refer to nipples as "perky," but mine are always protruded regardless of the situation.
Can exercise and weight loss improve this condition? Currently, what troubles me the most is the protrusion of the nipples and areolas.
Thank you.
Lin, 30~39 year old female. Ask Date: 2016/06/17
Dr. Wu Jianting reply Breast and Thyroid
Normal nipples are protruding.
If there are concerns about their shape or if there are suspicions of other issues, it is recommended to visit a clinic for a physical examination.

Gynecomastia, commonly referred to as "man boobs," is a condition characterized by the enlargement of breast tissue in males. It can be caused by various factors, including hormonal imbalances, obesity, certain medications, and underlying health conditions. In your case, it seems you are concerned about whether your breast changes are due to gynecomastia or simply obesity.
Given your height (165 cm) and weight (70 kg), it appears that you may be classified as overweight or even obese, depending on your body composition. Obesity can lead to the accumulation of fat in various areas of the body, including the chest, which can create a breast-like appearance. This is often referred to as pseudogynecomastia, where the breast tissue itself is not enlarged, but rather, fat deposits create a similar appearance.
The fact that you have experienced breast tenderness during puberty and have engaged in activities that may have affected your breast tissue, such as squeezing your nipples, could also contribute to the changes you are observing. However, the persistent protrusion of your nipples and areolas may suggest that there is more than just fat accumulation at play.
To differentiate between gynecomastia and pseudogynecomastia, a thorough evaluation by a healthcare professional is essential. This may include a physical examination, a review of your medical history, and possibly imaging studies like an ultrasound to assess the breast tissue composition. Hormonal testing may also be warranted to check for any imbalances that could be contributing to breast tissue enlargement.
In terms of management, weight loss can indeed help reduce the appearance of breast tissue if obesity is a contributing factor. Engaging in regular exercise, such as strength training and cardiovascular activities, can help you lose weight and improve your overall body composition. However, it is important to note that while losing weight may reduce fat deposits in the chest area, it may not completely eliminate gynecomastia if there is a significant amount of glandular tissue present.
If gynecomastia is confirmed and is causing you distress, there are treatment options available. These may include medication to address hormonal imbalances or surgical options to remove excess breast tissue. Surgical intervention is typically considered when gynecomastia is persistent, bothersome, and has not responded to other treatments.
In conclusion, the changes you are experiencing in your breast area could be attributed to a combination of factors, including obesity and possibly gynecomastia. A comprehensive evaluation by a healthcare provider is crucial to determine the underlying cause and to discuss appropriate management strategies. Remember that you are not alone in this, and many men experience similar concerns. Seeking professional guidance can help you find the best path forward for your health and well-being.
