Menstrual issues
Hello, Dr.
Chang.
My girlfriend started her period yesterday, but it was very light and then stopped.
Later, around eleven at night, she had a little bleeding again, which also stopped.
The next morning, she experienced a little bleeding again, which stopped as well.
Last month, we had intercourse but took precautions, and we also took a pregnancy test, which came back negative.
What could these symptoms indicate?
Haohao, 20~29 year old female. Ask Date: 2016/06/20

Hello, based on your description, the bleeding could be due to hormonal imbalance.
However, bleeding can also occur in the early stages of pregnancy or around implantation.
Testing for pregnancy too early may yield inaccurate results.
Therefore, it is recommended that if your period is more than a week late, you should check for pregnancy.
Thank you! Wishing you peace and health.

Irregular menstrual symptoms can be concerning and may indicate a variety of underlying issues. In your girlfriend's case, experiencing light bleeding or spotting that starts and stops can be attributed to several factors, especially considering her recent sexual activity and the negative pregnancy test.
Firstly, it’s important to understand that menstrual cycles can vary significantly from month to month due to a range of physiological and environmental factors. The average menstrual cycle lasts about 28 days, but cycles can range from 21 to 35 days. Irregularities can occur due to hormonal fluctuations, stress, changes in weight, exercise habits, or underlying medical conditions.
In your girlfriend's situation, the light bleeding she experienced could be classified as spotting, which is not uncommon. Spotting can occur for several reasons:
1. Hormonal Imbalances: Fluctuations in hormones, particularly estrogen and progesterone, can lead to irregular bleeding. This can happen due to stress, changes in diet, or significant weight changes.
2. Ovulation: Some women experience ovulation spotting, which can occur around the middle of the menstrual cycle when the ovary releases an egg. This is usually light and short-lived.
3. Implantation Bleeding: Although your girlfriend has taken a pregnancy test that came back negative, very early pregnancy can sometimes cause light bleeding known as implantation bleeding. This occurs when a fertilized egg attaches to the lining of the uterus, typically around the time a period would be expected.
4. Contraceptive Methods: If your girlfriend is using hormonal birth control, it can cause irregular bleeding or spotting, especially during the first few months of use. Even if she is using protection, hormonal fluctuations can still occur.
5. Medical Conditions: Conditions such as polycystic ovary syndrome (PCOS), fibroids, or endometriosis can lead to irregular menstrual cycles and spotting. If the irregularity persists, it may be worth consulting a healthcare provider for further evaluation.
6. Infections: Sexually transmitted infections (STIs) or pelvic inflammatory disease (PID) can also cause irregular bleeding. If there are accompanying symptoms such as pain, unusual discharge, or fever, medical attention should be sought.
Given that your girlfriend has experienced this irregular bleeding and has had a negative pregnancy test, it is advisable to monitor the situation. If the irregular bleeding continues for more than a few cycles, or if she experiences any additional symptoms such as severe pain, heavy bleeding, or other concerning signs, she should consult a healthcare provider for a thorough evaluation.
In summary, while occasional irregularities in menstrual cycles can be normal, persistent or concerning symptoms warrant medical attention. A healthcare provider can perform necessary tests, including blood tests to check hormone levels or imaging studies, to determine the underlying cause of the irregular bleeding and recommend appropriate treatment options.
