Want to get pregnant..?
Hello Doctor: I am currently 37 years old! I have been trying to conceive for six months and had a suspected ectopic pregnancy that required surgery two months ago.
However, the doctor confirmed that the surgery revealed no visible egg, so the ovary was not removed! Following that, I had a hysterosalpingography, which showed everything was normal and the fallopian tubes were open.
The follicle size was 1.8 cm, and the endometrial lining is sufficiently thick.
I have been tracking my ovulation during the ovulation period, but I still haven't been able to conceive! Could you please advise me on what additional tests I should undergo? Should I consider taking ovulation medications? Given my age, I would like to conceive as soon as possible! Thank you, Doctor.

Dr. Cai Yonglong reply Obstetrics and Gynecology
Hello: At 37 years old, you are considered to be of advanced maternal age, which can make conception more challenging.
It is advisable for you to consult a fertility specialist.
Taking ovulation medications is just one of the options, and it is also important for your partner to undergo an evaluation.

Struggling to conceive at the age of 37 can be a challenging and emotional journey, especially after experiencing complications such as an ectopic pregnancy. It’s important to approach this situation with a comprehensive understanding of fertility, potential evaluations, and treatment options available.
At 37, fertility naturally begins to decline, and this can be exacerbated by previous reproductive health issues. Given your history of an ectopic pregnancy and the subsequent surgery, it’s crucial to ensure that your reproductive system is functioning optimally. Here are some essential steps and considerations you might want to explore:
1. Comprehensive Fertility Evaluation: Since you have already undergone a hysterosalpingogram (HSG) that showed your fallopian tubes are open and an ultrasound that indicated a healthy follicle size, the next step could be a more detailed fertility evaluation. This may include blood tests to check hormone levels such as Follicle Stimulating Hormone (FSH), Luteinizing Hormone (LH), Estradiol, and Progesterone. These hormones play a vital role in ovulation and overall reproductive health.
2. Ovarian Reserve Testing: Given your age, assessing your ovarian reserve is crucial. Tests such as Anti-Müllerian Hormone (AMH) levels can provide insight into the number of viable eggs remaining in your ovaries. This information can help guide your treatment options.
3. Regular Ovulation Monitoring: You mentioned that you are tracking your ovulation, which is excellent. Consider using ovulation predictor kits (OPKs) to better identify your fertile window. Additionally, tracking basal body temperature (BBT) can provide further insights into your ovulation patterns.
4. Lifestyle Modifications: Ensure that you are maintaining a healthy lifestyle, as factors such as diet, exercise, and stress management can significantly impact fertility. A balanced diet rich in antioxidants, regular physical activity, and adequate sleep can improve overall reproductive health.
5. Consultation with a Fertility Specialist: If you have not already done so, it may be beneficial to consult with a fertility specialist. They can provide personalized recommendations based on your medical history and current health status. They may suggest treatments such as ovulation induction with medications like Clomiphene Citrate or Letrozole, which can help stimulate ovulation.
6. Consideration of Advanced Reproductive Technologies: If natural conception remains elusive, you may want to discuss options such as Intrauterine Insemination (IUI) or In Vitro Fertilization (IVF) with your specialist. These methods can increase the chances of conception, especially if there are underlying issues that are difficult to address through natural means.
7. Emotional Support: The journey to conception can be emotionally taxing. Seeking support from a counselor or joining a support group can provide a safe space to express your feelings and connect with others who are experiencing similar challenges.
In summary, while your age and previous ectopic pregnancy may pose challenges, there are numerous steps you can take to enhance your chances of conception. A thorough evaluation by a fertility specialist, combined with lifestyle adjustments and potential medical interventions, can provide a clearer path forward. Remember, it’s important to be patient and kind to yourself during this process. Each step you take is a move toward understanding your body and optimizing your chances of achieving your dream of pregnancy.
