Postoperative sensitivity and tingling in the glans area?

Hello Dr.
Chen, I underwent penile correction surgery (traditional circumcision) on June 3rd.
Since then, the pain in my penis has gradually improved, although there is a healing wound on the ventral side that had previously opened, and the sutures are still in place but starting to dissolve.
However, there is a region on the ventral side of the glans and the adjacent swollen foreskin that is extremely sensitive and painful upon touch, but I do not experience any particular sensation or itching when not touching it.
I am unsure why this area of the glans and part of the swollen foreskin is sensitive and painful.
Visually, the glans does not show any significant redness, swelling, or discharge, and the foreskin only exhibits swelling with a slight yellowish sensation but no discharge.
The rest of the glans and foreskin feel normal; it is just that specific area that is sensitive and painful.
Additionally, because it is on the ventral side, I have to wrap it while at work, as wearing pants causes unbearable discomfort.
I am uncertain about the cause—is it an infection or inflammation, a neurological issue, or a normal postoperative complication? Thank you for your assistance.

Post-surgery sensitivity and pain in the glans, particularly after a penile correction surgery involving circumcision, can be concerning and uncomfortable. Based on your description, it seems that you are experiencing localized sensitivity and sharp pain in the area around the glans and the swollen portion of the remaining foreskin. While it is encouraging to hear that the overall pain is gradually improving, the specific symptoms you are describing warrant further exploration.

Possible Causes
1. Normal Postoperative Sensitivity: After any surgical procedure, especially one involving delicate tissues like the penis, it is common to experience heightened sensitivity in the surgical area. This can be due to nerve endings being more reactive during the healing process. The sensitivity may gradually decrease as the tissues heal.

2. Swelling and Inflammation: The presence of swelling (edema) in the foreskin can contribute to discomfort and sensitivity. Swelling can occur as part of the normal healing process, but if it persists or worsens, it may indicate inflammation or irritation.
3. Nerve Regeneration: Following surgery, nerves may take time to heal and regenerate. This process can lead to sensations of tingling, sharp pain, or heightened sensitivity in the affected area.
4. Infection: Although you mentioned there are no visible signs of infection (such as redness, swelling, or discharge), it is still a possibility that should not be ruled out. Sometimes infections can present with subtle symptoms, and localized pain can be a sign of an underlying infection.

5. Scar Tissue Formation: As the incision heals, scar tissue can form, which may lead to sensitivity or discomfort. Scar tissue can sometimes be more sensitive than the surrounding tissue, leading to the sensations you are experiencing.

Recommendations for Management
1. Follow-Up with Your Surgeon: It is essential to keep your surgeon informed about your symptoms, especially if they persist or worsen. They may want to examine the area to rule out any complications such as infection or improper healing.

2. Pain Management: Over-the-counter pain relief medications, such as ibuprofen or acetaminophen, may help manage discomfort. However, consult your doctor before taking any medication to ensure it is appropriate for your situation.

3. Avoid Irritation: Since you mentioned that wearing pants causes discomfort, consider wearing loose-fitting clothing to minimize friction against the sensitive area. This can help reduce irritation and allow for more comfortable healing.

4. Cold Compress: Applying a cold compress to the swollen area may help reduce swelling and provide temporary relief from pain and sensitivity. Ensure that the compress is not applied directly to the skin; wrap it in a cloth to avoid frostbite.

5. Gentle Care: Maintain good hygiene in the area, but avoid aggressive cleaning or manipulation of the sensitive areas. Gentle washing with mild soap and water is usually sufficient.

6. Monitor Symptoms: Keep an eye on your symptoms. If you notice any changes, such as increased redness, swelling, discharge, or fever, seek medical attention promptly, as these could be signs of infection.

7. Consider Physical Therapy: If sensitivity persists, discussing the possibility of physical therapy with your healthcare provider may be beneficial. A physical therapist can provide techniques to help desensitize the area and improve comfort.

Conclusion
While postoperative sensitivity and pain can be a normal part of the healing process, it is crucial to monitor your symptoms and maintain open communication with your healthcare provider. They can provide tailored advice and interventions based on your specific situation. Remember, healing takes time, and it is essential to be patient with your body as it recovers from surgery.
