Post-molar evacuation?
Hello doctor, I had a dilation and curettage (D&C) procedure for a molar pregnancy on May 31.
My first period started on June 29, and the flow was light at first, but on June 30, it suddenly became very heavy, almost like a hemorrhage.
The sanitary pad was almost completely soaked within 2-3 hours, and I felt a bit dizzy from the blood loss.
Is this normal for the first menstrual period after a molar pregnancy procedure?

Please return to the clinic for follow-up on the hCG levels to determine whether this menstrual flow is normal.

After undergoing a dilation and curettage (D&C) procedure for a molar pregnancy on May 31, it is understandable to have concerns about the subsequent bleeding and menstrual irregularities. Molar pregnancies, which are abnormal growths of trophoblastic tissue in the uterus, can lead to various complications, including significant bleeding, especially after surgical intervention.
Your experience of having a light flow on June 29, followed by a sudden and heavy bleeding episode on June 30, raises several important considerations. It is not uncommon for women to experience irregular bleeding patterns following a D&C, particularly after a molar pregnancy. The first menstrual cycle post-D&C can vary widely in terms of flow and duration. Some women may experience heavier bleeding, while others may have lighter periods. However, the sudden onset of heavy bleeding that fills a pad within a couple of hours is concerning and warrants immediate medical attention.
Heavy bleeding after a D&C can be attributed to several factors, including retained products of conception, which may occur if some tissue remains in the uterus after the procedure. This retained tissue can lead to continued bleeding and may require further evaluation or treatment. Other potential causes of heavy bleeding include hormonal imbalances, especially if the body is still adjusting after the molar pregnancy, or complications such as infection or uterine perforation, although the latter is less common.
Given your symptoms of feeling faint alongside the heavy bleeding, it is crucial to seek medical attention promptly. Heavy bleeding can lead to significant blood loss, which may result in anemia or other complications. Your healthcare provider may perform a physical examination, ultrasound, or blood tests to determine the cause of the bleeding and ensure that there are no retained products of conception or other complications.
In summary, while some irregular bleeding can be expected after a D&C for a molar pregnancy, the severity of your symptoms, particularly the heavy bleeding and associated dizziness, should not be taken lightly. It is essential to communicate these symptoms to your healthcare provider as soon as possible to ensure appropriate evaluation and management.
In the future, if you experience similar symptoms, such as soaking through a pad in an hour or less, passing large clots, or feeling faint, it is important to seek emergency medical care. Monitoring your symptoms and maintaining open communication with your healthcare provider will be key in managing your recovery after a molar pregnancy and ensuring your overall reproductive health.
